Toward the end of the sequence on the surface of the SB's ship, during which all the characters are wearing their hazardous environment masks, Liara's disappears, while the others' remain.
After Shepard reminisces with Liara on the Normandy, and you speak to her at her new digs, her head doesn't turn to align with Shepard's, nor does Shepard's turn to align with hers. She stares straight ahead, as does Shepard. It's unsettling!
To clarify the second point, all the npcs I've noticed in both DA and ME2 turn to face the PC when they're walking past, or when speaking to them outside of the cinematic dialogues.

Aside from these extremely minor points, it really is superbly done and I applaud you on it.
