Okay, I've begun to notice that for some reason Squadmates are only appearing with 28 points for skills instead of 30 or 31.  Also with the arrival of the Shadow Broker DLC I noticed some things:

I had two characters that beat the game before the second patch came out.  My first character was an Infiltrator and the second was a Vanguard.  Both characters played the majority of the game on Normal/Veteran difficulty.

The Infiltrator was able to reach level 30 and was able to allocate 30 points for Kasumi.  In fact this character was the first one to play the Kasumi DLC when it was first released.

The Vanguard only reached level 29.  I recently played the Kasumi DLC and when I recruited Kasumi I was only able to allocate 27 points.  After completing the Kasumi mission I was able to level to 30 and Kasumi received an extra point.  Shepard was able to have a total 51 points.  Other characters that I had previously recruited only had 30 points (31 for Miranda/Jacob)

Something about this game doesn't grant squad points properly if you recruit and/or gain loyalty after a certain level.  If you're near level 30 by the time you recruit someone they only come with 28.

Here's where it gets interesting:  The squad retrain console in the Shadow Broker DLC resets a Squadmate's total to 30 if they are loyal.  So worse case scenario, you just retrain them if they are short points and it seems to fix it.  Although that costs EEZO to do.

Anyways I hope this helps BIoware troubleshoot the issue, although I don't know if they're going to do anymore patches.
