I just finished horizon and am halfway through lotsb on insanity. I was worried that ammo would be a big problem but its not at all, im finding looking after the squad more of a challenge. Seeing them run past you when your in cover is like NO GET DOWN. Placing them in the right place is more of a challenge, a few times if one has died iv left them dead until the fight is over just to save on medi-gel lol. I haven't ran out but its that 'you don't know what could happen round the next corner' that makes me hate using it too much. The fighting is much more fun all round though.

IMO the hardest possible NG+ mission is anything BUT Kasumi's, because it means you're doing it without a Locust. :P

Yes, but Locust is hardly primary weapon on a NG+ playthrough.

... the locust is always my primary weapon? except on a vanguard I guess.
but unlocking flash and nabbing the kasa locust makes kasumi's loyalty a top priority every playthrough. A good tip is to know the mission order for getting tech/ weapon upgrades quickly.

on sentinels and engineers I always go Saronis Applications -> Kasumi -> Mordin -> Garrus. boom, cooldown and duration bonuses before horizon, easy as.

This is NG+ we are talking about. You have AR/SR/SG and/or Widow/Claymore/Rev .. why would you use Locust over Evi, Mattock, Viper or the big three unless you ran out of ammo?

IMHO NG+ difficulty is largely overrated. It is not much harder than vanilla Insanity. Sure, you will be less effective for like 5 or 6 hours into the game. After Horizon you can easily get all the important upgrades and since then there is no difference at all. Not to mention that every DLC brought some upgrades (extra biotic damage, tech - this means you get CD upgrades sooner).

Actually NG+ can be beneficial for some classes like Vanguard. You have lower cooldown thanks to maxed passive and Heavy/Area Charge right from the start + Inferno and squad Cryo ammo. The only difference is that you might have to melee more until you get SG damage upgrades and the shield/barrier piercing upgrade (like 3h in tops).

You also have advanced weapon from CS right from the start no matter what class are you.

Same for Soldier, Adrenaline Rush has the same cooldown for the whole game. You have ammo against everything. Can't be easier really.

Adept (and maybe Engineer) is perhaps the only class where lack of upgrades make significant difference.

And if you have some crazy bonus power like Energy Drain, Reave and now maybe even Stasis it is just as easy as vanilla Insanity. Just my experience though.

I agree about the stasis on a NG+ Adept.  I'm using it plus the mattock and having no problems.  It only took three shots to kill the Krogan on Mordin's recruitment.  I also took the shields of the Ymir on Freedom's Progress in under 5 seconds.  Neither Miranda or Jacob went down.  Stasis definitely give reave a run for it's money... though I still prefer a vanguard with reave.

My [very general] recommendations are:

-- Know yourself ... I.E. what is your classes strengths and weaknesses. Play to your strength and try not to expose your weakness much.

-- Know your enemy ... Know what types of enemies you'll face [esp. what protections they have such as shield, armor, barriers or some combo].

-- Know your locations ... on insanity it's good to use the knowledge gained from prior playthroughs to minimize the hair-pulling on your end. Knowing on certain levels that you face an infinite number of enemies until you do XYZ is important ; knowing how many waves of enemies is important --- I.E. you don't want to be running around, kill the last guy in a wave just to get ambushed as the next wave spawns on top of you. Basically know when, where, and about how many enemies will be coming at you and get into the cover that best suits what the enemy will try to do against you.

-- When in doubt try to fall back before it's too late

-- Don't be afraid to use medigel if you're hurt

-- Try to avoid shield-boosting skills [Fortification, Geth Shield Boost, and Barrier]. The cooldowns are too long and the protection that they give you is minimal. For the most part medi-gel is a much better solution. There are usually much better bonus skills ... Reave, Energy Drain, and Warp Ammo all come to mind as being situationally very useful.

-- Don't be afraid to respec Shep's points or change his bonus power as the mission demands. Energy Drain won't do anything against Collectors but Reave or Warp Ammo may work wonders. Sometimes even respeccing a passive is good if you need higher Paragon / Renegade points for a check that you know you will need to make.

-- Your classes "power skill" should be spammed when you have a reasonably good opportunity to do so ... Adrenalin Rush, recreating an expired Combat Drone, Tactical Cloak, Singularity, Pull, etc. are all skills that should get cast a lot. Try not to cast other powers with long cooldowns like Fortification as they'll get in the way of you using your "power skills".

-- Don't waste Shepard's points on ammo powers that your teammates can provide just as well as he can. For example there's no reason to take Squad Disruptor or Incindiary with Shep if you can just get Zaaed / Grunt / Jacob / etc. to take the power and bring them when needed. This means Shep can spend his points on other powers that squadmates can't get [Squad Cryo] or Shep can take the more damaging evolved version of the power [like inferno ammo].

-- Take along the right teammate for the job:

... Zaaed & Squad Disruptor work well vs. Blue Sun && Geth heavy levels

... Grunt works well on Husk && Blood Pack Heavy Levels

... Jacob works fairly well as a Grunt replacement esp. if you intend to set up biotic combos

... Miranda is almost always welcome as she is quite effective vs. almost any enemy .... Husks excluded

... Garrus is decent vs. Blue Suns and possibly vs. Mechs with Squad AP Ammo

... Tali & Legion are good vs. Geth .... not so hot in other missions

... Kasumi can glitch Harbinger so she is esp. handy here

... Adepts can make use of Thane & Samara but I haven't found them great with other classes

... I don't really see much use for Jack or Mordin as you can often find better combos of squadmates to use

Mordin has the fastest cooldown crowd control of any squadmate with his Full Cryo Blast.

Thane and Samara are both great in Collector missions.

Loyal Tali is good in merc missions as well.
