Just make sure you do Miranda's loyalty right after Jacks (or vice versa), you have a better chance to get both their loyalty after the fight. I usually keep their two missions till towards the end (as well as Tali's).

I think you have to have your paragon in the last bar or else you'll lose loyalty with one of them you can make amends afterwards if you talk to them in their quarters aswell though

I don't think it matters if the bar isn't that high, it's best to do the two missions as close to each other as possible. It's extremely hard to regain loyalty when it's already lost. You just end up falling in a burning ring of fire...

Actually I've read where the renegade/paragon bars don't really have anything to do with it.
(can't remember which thread explained it best though)
IIRC - basically what was said, the game keeps track of how many times you use the blue/red dialogue options beforehand, to unlock them in the dispute.
If you're having problems now, and have a save before Miranda's loyalty mission (I believe you said you did Jack's first) load it & do 2 or 3 missions where you can pick the blue/red options as much as possible (your choice).
I would suggest recruitment or loyalty missions (they give you the highest number of blue/red choices).
Then go back to Miranda's loyalty mission and it should work now. (save before - just in case it didn't work yet)

It isn't aout amount of Paragon points at all.
Read this - http://social.biowar...5/index/2990124 It actually has nothing to do with the amount of Paragon points you have, it is about doing all the Paragon options in an area at one time and doing the hard decisions, like Jack & Miranda fight, early on, having missed no potential paragon ppoints in any area you have visited.
On my 4th playthrough I tested this out, minus editing the saved files part. Worked like a charm! I just played it straight, finishing all Omega first, not visiting the Citadel at all until I had done all of Ilium and Miranda and Jack's Loyalty missions, to keep the proportions of Paragon/Renegade possible points I had missed as low as posssible. I have done the Collector ship, the Shadow Broker, but still had loyalty for Jacob, Grunt, Garras, Thane, Samara, to do. Also not done Normandy Crash site, Kasumi, or Overlord yet. The fight with Jack & Miranda gave me the Paragon Interrupt no problem, the one that keeps them both loyal.
The next tricky one is apparently a Tali and Legion stand off. I have never had a problem with it though. But you should not get Legion till you are about ready to do the IFF and Suicide mission because as soon as you do the IFF it triggers the Collectors taking all your crew withing 2 missions I think it is. You have time to do Legion's Loyalty and that is it ,or you lose all your crew - folk like Kelly & the 2 fun engineers etc. You can rescue them all IF you go after them immediately.

Now I have done the Thane and Garras loyalty missions, and my Paragon bar is full to the top!

It's all about the ratio of points gained versus points available. You can meet the persuasion check with a bar only half full or you can meet it with the bar completely full. And you can fail the persuasion check even though the bar is full. For the default game or new game plus playthroughs, you're starting with no points and have to be consistently paragon or renegade -- no splitting up your decisions half and half there. If you import an ME1 save, you can start off with up to 190 points in paragon or renegade. This has usually been a significant point boost to allow people to be a little less stringent on their morality choices while still meeting the check.
The Miranda/Jack loyalty mission is easier to resolve as a Renegade. The Tali/Legion loyalty mission is easier to resolve as a Paragon. They're both easier to resolve if you do them earlier because the ratio between points gained to points available will be better.
And if you can't get their loyalty in this game, remember you can still bring everyone out of the suicide mission alive even if you don't have all their loyalties.
The Miranda/Jack loyalty mission is easier to resolve as a Renegade. The Tali/Legion loyalty mission is easier to resolve as a Paragon. They're both easier to resolve if you do them earlier because the ratio between points gained to points available will be better.
