How hair doesn't seem to flow or move, its just a solid piece of wig and no matter how you toss and turn its not going to flow according to the direction of movement.

The way (NPC) people's mouths that populate the worlds never move when they are talking. Though they gesture and move like they are conversing. I never know who exactly is talking at first glance. People's mouths moved in ME1, but not in ME2. It does this on both PC and console...

The animations of the SMGs.
The animation for the Shuriken is correct, seeing how it's a handgun. However, the Tempest and the Locust have buttstocks and vertical foregrips; they were clearly intended to be fired from the shouldered position as you would a rifle or real life SMG like the Colt 6450 submachine gun, or the H&K UMP45. However, everyone holds them in a two handed pistol grip instead.

My tiny complaint is with the engine, Gameplay tiny things like getting stuck in cover while running by an object.(Not trying to take cover.) Auto-Aim Targeting other enemies then the one I'm shooting at. Biotic powers flying by previously targeted enemies,or flying by them and looping back around. Squad and enemies charging into bullets. Why would any one run face first to their death?

The way FemShep walks, sits or does anything at all. She is a woman she should animate like one.

fshep not moving or sitting like a woman like she did in ME
All the npcs standing there and the same npcs complaining about X 24/7
Jack or Mornith even being allowed to be on the ship crazy =/= good in the combat area
Kasumi should've been excluded from the game I like her but a thief on a ship on a SM? WTF? Jack should've been excluded to getting a crazy biotic was somehow better then getting an uber powerful stable one why?
Shep being a brick 95% of ME2 (though he is gaining points for the DLCs)
Fshep boxing with a yahg I'm sorry metal bones still can't cover that one for me its too damn strange looking.
shep having zero trauma about being brought back from the dead. Shep having zero questions beyond was there any other test subjects? Not even entertaining or making a mental note to ask Miranda on the subject after EDI reveals they put reaper tech in her? I mean wouldn't you want to know if you're part reaper? Really no shep...ok.

Has probably been said but I am constantly annoyed that no one thought of putting proper PC controls and shortcuts into ME2 (they were there in ME1). No mouse wheel, no direct access to journal/codex/squad screen, etc. Couldn't have been that difficult.
This.
