I ran around Illium for a good 30 minutes trying to find Liara's office. It was pretty sad.
And apparently you can see the tech specialist in the pipes? I'll check that out on my next playthrough.

One moment I can think of off the top of my head is when I came across my first data pad. I quickly skimmed through the instructions, and thought this should be easy. After spending 2-3 minutes mashing the arrow keys, and clicking the mouse to no avail. I finally gave up and started looking around online for how to conquer the menacing device. After another 2-3 minutes of researching, I found my answer. Not once had I thought to try the movement or use key.

Finding out stasis also knocked people down when it wore off. When I first saw I was like, "What? Did Tela just trip?" And when I realized I proceeded to spam level one stasis on her, and while she was down I just kept smacking her with my rifle.
Also using concussive shot on her while she's charging is hilariously gratifying.

ME1 throwing grenades at thorian infected dudes and watching the colonist counter dwindle ...I thought the grenades would automatically be upgraded to the sleeping gas or whatever....
throws grenade... boom.."Well ...he don't look awake anymore.." looks at counter..."Waiiiitttt"
Pretty much every game I've ever played: Remapping the keys to my own prefrence, only to realise tutorials mostly don't take that into account..... "IM PRESSING E!!!"

In ME2:
-during first playthrough, not going to the Omega-4 relay right away, and killing off half of the crew.
-Sending Zaeed as fire team leader, even though he lost control of the gang he himself started
-Remapping the controls to the hammerhead the first time i played it to make it easier to use, then forgetting what i mapped them to the next time i went to use it.
(this made me mad for a bit, then felt like a complete dolt when i realized why the controls weren't working)
-used the Cain to wipe out a tough group of enemies, then not having enough ammo to use it in the next room's boss battle (whoops, never made that mistake again)
-Not seeing that Shep had a "skylight" above the bed.
-doing every side-quest available prior to aquiring Legion for the team, guarantying I'll never hear all of it's dialog.(i still havn't heard all of it's dialog, btw)
-not realizing until playing this afternoon that I can just stand behind joker and he'll start talking at random about things. I've played this game about 6 times at least across both pc and 360. (feels like dumb***)
