Sometimes they'll make you press a button to get in cover. I find this annoying. But if you don't have to press a button then you can end up in cover by mistake.
Gears of War is probably the first game where I remember thinking "hey, this cover system actually kind of works!".

Until you get the hang of it, try using the tactical pause after each Charge. You can line up shots, plot your escape route, and assign commands without worrying about dying. Also, you shouldn't be sprinting to cover after a Charge if it's well planned, you should be walking or vaulting. And until you get the time dilation (Heavy Charge), you need to be extra careful.

change the line

to

and create the line
Bindings=( Name="Shared_Storm", Command="OnRelease StormOff | OnHold 0.2 StormOn" )
In the section [SFXGame.SFXGameModeDefault],
Bindings=( Name="X", Command="Shared_Storm" )
Pressing my normal Shared_Action key, SHIFT, worked for taking cover but didn't work for storming, and storming to cover with the new storm key made Shepard take cover. :/

That can be done. In [SFXGame.SFXGameModeBase] create the line
Bindings=( Name="Shared_Vault", Command="TryClimbOrMantleCover" )
and remove TryClimbOrMantleCover from the line
Bindings=( Name="Shared_Action" ... etc )
Then bind Shared_Vault to a new key in the section [SFXGame.SFXGameModeDefault]
Bindings=( Name="X", Command="Shared_Vault" )

Playing on xbox - what happens is that you move behind cover ( but not in conver ) and while you take a lot of fire you press the A button to get in cover, which does not happen immediately so you press A again - then both A button presses process and you vault to your death.
Eventually I got used to always getting in-cover when I saw some cover, and now it almost never happens. If there is a dev looking at this please don't change it now that I am used to it.

The binds in the in-game menu override binds in the [SFXGame.SFXGameModeDefault] as far as I know, so to make something in that section work, you have to make sure the key is clear in the in-game menu.

95% of my Soldier deaths. No lie. 1% actually dying, 4% self-nuking via Cain.

makes it very hard to run in illium or citadel without selecting all those annoying adds. Still, with time you get used to it

I'm glad other people list their number one cause of death as 'unintentionally leaving cover'. I feel slightly less of a rook. On xbox it is indeed a pain.

Nice, I'm with OniGanon - I too die occasionally when pressing the spacebar too liberally when Shep doesn't take cover immediately. It's really annoying to watch Shep jump over instead of taking cover and dies
