I resurrected by first Mass Effect game, a Paragon Infiltrator Shepard, to tackle the game on Insanity. I'm going the "New Game Plus" route. I'd like to hit level 60 in addition to tackling the game's hardest difficulty level, but when I finished my first play-through on this character, which focused mostly on the campaign missions and didn't finish all of the assignments, he was only level 42.
Is this possible? Can I hit 60 when starting a new play-through at 42? Or is the last gap in experience too wide?

The last gap in experience is going to be too wide to get from 42 to 60. It's rather difficult to achieve even from level 50 to 60. When people want to reach level 60, I suggest doing speed runs (main quest only), which can be done in less than 10 hours. Starting about level 53, I think a more thorough runthrough will get them to 60. Even easier if you can get to level 57 because the amount of experience you gained to get to level 57 is near enough to the experience gained from level 57 to 60.

I reached Level 50 on one playthrough. Using the same career on the second playthrough took me to level 58 (I think) - and that was with all the assignments. The third playthrough finally got me level 60.
Insanity is annoying - not because it's hard, but because all the Mercs keep using Immunity and it takes a really really long time to kill them, even when your using Level 10 spectre gear and level 10 weapon and armour mods.
I remember hitting one Krogan multiple times with the Mako's cannon and the f***er still didn't die.

Sad to say, the enemies aren't smarter, they just have a BUNCH more HP and love to spam immunity. It is the reason I don't play insanity much.

Master spectre sniper and master assasination make short work of most organic enemies even on insanity.
Plus dont forget to give your companions gear of similar quality.

This also changes if you've already reached lvl 60 before, since you get the additional 5% exp bonus after that, so getting additional characters maxed out is easier.
