Yes, this is where everyone can share their most boneheaded moves, strategies and sins of omission while playing Dragon Age. All are welcome, no one is judged.

I'll start:

After carrying Yusaris (20% fire resistance, +10 damage vs. dragons) around for, I don't know, EVER, I traipsed off to face the High Dragon and failed to equip Sten with it. He didn't even equip the Genlock Forge Master's Hammer, which at least carried a bit of fire resistance and a lot of armor penetration. No, he took Her Highness on with his old friend Asala.

*hangs head*

I'm considering redoing the fight just to erase the ignominy, but we did get her on the first try and my PC got the melee kill, so maybe I should just let it go.

First run through - no points in coercion.

Me : "How the dickens do I get this blacksmith to let me inside?"
Owen : "Bugger off"

I was most of the way through my first game before I learned how to choose specializations, and it wasn't until my 2nd game that I realized followers had them.

It took me a while to figure out how to get to camp.

My first time in Denerim, it took me about a half hour to figure out how to leave.

Ooh. How did the Defense of Redcliffe Village fight go?

My Mage did have enough Coercion to get Owen working again, but not enough to convince Dwyn to join the defense or to get the Reverend Mother to provide fake talismans to Ser Perth and his knights. In Dwyn's case, she tried first to Persuade and then to Intimidate him --hey, it worked for both of my Rogues!-- but *ahem* instead of being cowed, Dwyn took offense and she was forced to kill him in his own house. Oops.

OMG yes!  I got so frustrated there was language and gestures.  I think I ended up googling it or something....

I believe that marked the end of my first run though.  Oookay - coercion.  Check.

I don't think I even found Dwyn for two more runs... then I discovered the wiki ;-)

I have many, many bonehead moments from my first playthrough. I'll try to save some for later posts.

On my first playthrough, I gave the mages in my party daggers as their secondary weapons and set their tactics so that they would switch to them if attacked by a melee attack. I was so confused when I saw Wynne and Morrigan constantly swing their daggers and miss all the time. Of course, I also had no idea what the "attack" score represented...

I didn't realise you could rotate the head in the character creator at the start of the game, so for about ten runthroughs I had characters who looked great from the front but rather odd in profile.

I once was fighting the Archdemon with my official play-through, and almost had Morrigan die from the shadowy vortex attack once. Thankfully one of my most forgetful and "blonde" moments were not to bad, for I managed to save her countless times with poultices.

Eh I'd also forgotten once that my Warden had forgotten to persuade Alistair on his first play-through to forgive him for having sacrificed Isolde to save Connor. Because of that I never got the fourth friendship bonus from that ungrateful whelpling. Luckily I convinced him the second time to cut me some decent amount of slack that I managed to achieve the final level of friendship with him. My first play-through with this however was indeed a facepalm moment.

I would say my biggest facepalm moment was during my first play through getting lost in the wilds not realizing that there was an arrow on the map pointing to where I needed to go *sigh* I was in there for a good few hours...
I don't know how many darkspawn blood vials I got but it was certainly ALOT

I didn't know until after completing two playthroughs that there was a tavern/inn in Redcliffe.

My first playthrough featured a number of "unprepared" moments --like, where I picked traveling parties based on the types of banter they might have, instead of what would actually make sense; or, thinking to myself, "Well, I'm just going to Denerim to drop off some Drake Scales/going to the Spoiled Princess to look up Oghren's old flame/etc., there SURELY won't be any fighting so I don't need to bring a healing Mage or any number of health poultices to speak of" [cue crossing-of-swords noise indicating random mid-map battle encounter. . .Pigeon crap.]

When the shriek ambush first happened at camp, many people had no tactics set up (because I never had them in the party), and Sten didn't even have equipment. I think my PC and Leliana were the two people left standing, with me fleeing in circles with a tiny bit of health while a shriek chased me and Lel took pot shots.

Good times.

I did the same exact thing. To add insult to injury, the way I found it was on these forums. Someone mentioned kissing the barmaid. Even once I knew there was a pub, it took me forever to find it in game.

I thought I had a cute Dwarf for my first run. I didn't.

I thought I had a cute Dwarf for my first Awakening run (Different character). I didn't.

Hmm... Let's see... I think it was probably something to do with lockpicking. So I just downloaded Lock Bash :D

It didn't occur to me to have my "I'll steal from anyone" Rogue attempt to steal from Caridin until after she had already initiated the dialogue that ended with him throwing himself into the lava. If I'd stopped the game and reloaded I would have had to do the whole Branka/Caridin/Shale/Oghren conversation and fight over again. *facepalm*

-couldn't convince the Reverend Mother to free Sten. Did not know you could just unlock the cage yourself, so Sten died in Lothering. D:

- Didn't know the Chasind trail signs was some sort of quest until my third playthrough.

- Didn't know you had to leave Alistair at the gates if you wanted to do the US. I thought it had to do with my "Persuade" skill.

- Didn't know you could help out the dude that was being threatened by the carta thugs at Orzammar so I usually just walked by and treated it like scenery.

- Didn't know you could get Anora to talk about Loghain (not so much facepalm but rather missing interesting content).

- On my first Dalish playthrough, spent an hour looking for Tamlen. One internet search later find out that there is no way to save him and I am a big idiot. [Also got severely depressed over this fact.]

Trying to solve the flame puzzle in the Stone Prisoner DLC before discovering how to press 'H' to hold the party. But this also made me better at team movements....

The Landsmeet on my second playthrough, was going to try to get Loghain. Only I somehow hit on the right dialogue points that caused Alistair to kill Loghain, take the throne without my HNF and dumped her on her ass afterwards.

I didn't bring any ranged characters or rogues to the archdemon. When he flew away it took a loooong time for my mage to whittle it down with the basic attack and arcane bolt.

First time fighting the rotating head thing too, attacked all of the spirits.
