So the issue I'm having is explained below, but can be summed up in this video:

The issue is that I can't swap out party members properly. If I remove Sten, and replace him with Leliana, he disappears and Leliana never shows up. If i remove Morrigan and Alistair they leave like they are supposed to. When I bring them back they come back, but have no unit frames. The video explains it better than I can, which is why I included it. This is a second toon that I'm working on, I have already beat the game once, and never had this issue before. Any ideas on what I'm doing wrong?

I should also mention that I installed the toolset last night after beating the story. I reloaded a previous save of my first character and am now having this issue on him. The problem is a bit weird in some places. I feel these weird issues are somewhat irrelevant to contributing to the problem, but if you want a vid of what is going on I can post that too.

Sigh, I think the toolkit broke my game. I may just have to re-download it. It especially sucks because I only have 6 Mbps connection.

I have the same problem. It just started acting like that all of the sudden, and it has affected all my saved games.

Same here. loaded a save and when i got back to camp it was totally empty. Leaving the camp would prompt me to the char select screen, I choose a party but when I arrive at my destination I'm alone. Using older saves produces the same problem.
I started a new game, got to Ostagar, and for the '3 vials of blood quest' Alistair didn't join my party and now is nowhere to be seen.
I'm on a fresh Win7 install, i'll try removing the toolset and the DLC and see what happens.

Another thing you can try is just to remove the place between Single and Player in your Documents\\BioWare\\Dragon Age\\modules\\SinglePlayer folder. This seems to have helped for some people.

Tried, didn't do it for me =/ . Also tried deleting the folder, no fix.
Removed the toolset, restarted. still the same problem. Going to do a full reinstall now as a last resort.

Awesome thanks for those tips. Crimsoncobra57 sent me to this thread. I was having a problem coming out of combat during the Mage Origin. So I couldn't complete it. These steps oddly fixed them.

I uninstalled before I got any updates here, but I got it reinstalled now. When I loaded back in to test it (before trying the fixes here) I found that the game didn't fix itself in anyway. After reading what needed to be changed, why would it (lol)? Thanks for helping me out, the changes did indeed work.

Chandler, step 3 would be unnecessary because in the step before you are having me delete the root folder. Unless of course I am reading it wrong, which is possible.

I just wanted to bump this as it fixed my problems... even though my symptoms were different.
After installing the toolset:
1. None of my party members AI would do anything. They would only follow direct commands regardless of tactics or behaviour selected.
2. I could not exit any area. e.g. if I was in camp I could not go to world map.
3. I could not give gifts to my party members. Right clicking and selecting gift would do nothing.
After deleting the folder mentioned previously in this thread everything started working as expected.

um, I have this problem with a vaguely evil human female mage. alastair won't speak to me, the cutscene with that mage never happens, and I have never downloaded the toolset.

argh! restarted a new elf mage and exactly the same thing happened. all I can think of is this has only started happening since I patched to the latest version

So I had this EXACT same problem - and tried some of these fixes. Unfortunately, I have a lot of things in the packages/override and didnt want to delete them all... my Single Player folders were just an empty folder structure.
I copied out the packages/override and started to add things back in, first the pretty harmless things (new hair models, etc.) and eventually, it boiled down to just the "BETTER COMBAT" mod suite (better archery & better melee) and these files:

module_core.ncs
player_core.ncs
rules_core.ncs
If I leave them out of the packages/override, I don't have the party swap issue. But if they're present, I do. The modder who released that suite (I believe "Selea") seems to have disappeared from the site I got them from, but hopefully someone with more skill than me can help to solve this for everybody, so that we can enjoy the Better Combat mod while still enjoying a functional DA game.

Just to add some more information, trying to help everybody else out, it seems to be specifically the "module_core.ncs" that is the problem, which, fortunately, comes from:
All_talents_not_deactivated_after_combat-432.zip
an optional part of the mod... so I guess, this is ok, for now! But I fear there will be problems down the line..

Mr_Toad: Tried getting rid of module_core.ncs but still having that problem. Any idea if it's a specific thing inside module_core.ncs that could also be in other places? I am using Combat Tweaks not Better Combat mod.

fixed mine by removing everything in packages, modules, and addIns (except official) and adding mods back in one by one... found out that two mods had been installed twice for some reason, maybe a bug with DAModder
