I was on EA help, and explained it in a chat box to technical support person. They told me to sign the lil tab in the inner cover where the main game serial number was written and to take a picture of it, and attach it in the convo box and send it. After confirming i was telling the truth they sent me the code.
However, this ended up pointless as i was unable to get any dlc free. I thought you would get at least something minor free but it just gave me the chance to buy dlc. Maybe this is what's supposed to happen or maybe it was only free until a year after the game launch

I got my me2 + gta4 for £20 so the prices are coming down quite quick!

I've bought EA classic Mass Effect 2 from DimensionPlus.nl, Recieved it yesterday.
And as like many other people I didn't get my cerberus network code in my case either.
5 minutes chatting to a Live Support member resulted in activated DLC,
I've even asked if he/she needed proof, but he says "No, I see the code(s) you're using."
So that was resolved very quickly.

Just chiming in to say yes, I just got cerberus network activated for my account about two days ago. I got the ea classics version ME2 and there wasn't a CN code in the box - I contacted EA Customer Support about the issue and it was solved in about a day.
I didn't get an activation code, the CN access was added to my EA/O account directly, so I could log in with that within ME2.
I sent the complaint in the "Billing/Purchasing -> Other" category and I also had the game added to my Origin library before - just saying, in case these are relevant.
