This is the mission on the colony of Franklin where you have to choose to save the colony or the spaceport.  I just noticed that the record in my journal says the opposite of what I had chosen.  I chose to save the colony.  The end of mission report says I saved the colony. When I look at the in the journal, it says that I saved the spaceport.  I ran the mission again choosing the other option and again it recorded the opposite result in the journal.

This is a known issue. Apparently the game records your real decision, it's just the journal that's wrong.
