It works like this:
Backstabs will occur 100% if you are against an enemy(even if face to face) while still in stealth.
Whether a backstab will work or not then completely depends on whether you are detected or not when you move into the hexagon right against an enemy, be it behind the enemy or in front of the enemy. Every time you move along an enemy(moving from one hexagon against the enemy into another hexagon against that enemy), there is a chance that your stealth will break. Every time an enemy walks up right against you, there is also a chance that your stealth will break.
There has been no information yet on whether it increases your chance of getting detected if you walk straight into someone's face, as opposed to approach the target from flanks or behind.
A good idea is: do not attack an enemy straight away with one click to attempt your backstab in stealth. You should first move your character to the hexagon from where you plan on backstabbing the enemy. If the enemy detects you, you can then use a mighty blow or other ability to attack him instead, since you are already detected and cannot attempt backstab; if you are not detected, then you can have a 100% backstab.
A very important note which many people may not realize: special abilities such as Mighty Blow, Below the Belt, Deadly Strike etc. will NEVER proc a backstab, even if you remain undetected right against the enemy ready to stab. However, buffs such as Powerful Swings will contribute to your backstab damage. Do not activate those buffs while in stealth though, as that will break your stealth. You should always activate buffs first, then go into stealth.
