No one, I was on easy, and manage to save everyone on the first playthrough, but on my second playthrough on insanity, Mordin kept on dying on me, even when I completed his royalty mission.

Legion. Made the mistake of choosing Samara for my fireteam leader. Poor bot took one right in the dome as the doors were closing.

With games like this if I do only one run completing all the side missions it'll be the first unspoiled run. Everyone made it, loyalty missions done and decisions made based on what makes sense. I was honestly a little disappointed, I thought "Suicide Mission" meant someone would die anyway.
