On my first playthrough, I unexpectedly lost Zaeed on the Distraction Team. I didn't know he'd die, so it was very shocking when he did. That moment made the game for me, especially since Zaeed was my favorite character, and his death was so sudden.

luckly for me not one single person died on my first playthrough dont see why everyone had problems.but i did read some where after the last update,that they change up the outcome for characters dying or living through the suicide mission

My first and so far only playthrough(I need to finish my second playthrough of ME1 to import) I didn't have Zaeed or Kasumi and I saved all the crew and all my squadmates. I kinda cheated by reading a guide though. I'm definitely not going to read a guide or reviews or most previews/trailers for ME3 since the first two games I read alot of spoilers.

Jacob, I was naive enough to think he was able to handle the vents. Mordin could not hold the line.

The only time anyone died was when I did my sucide run PT, everyone died.
Other then that, I have never had anyone die, even on my first PT. Very disappointed. I expecting someone to die no matter what.

Jack on the Distraction Team on my first play through. I didn't like her much so I didn't care that I lost her just that I did lose a teammate. I thought she could hold her own.

Nobody did. It was pretty obvious how to keep everyone alive. I didn't use any guides or get any advice on how to do it.
The game basically tells you that you need them loyal to live.
Cannon, Armour and Shield Upgrades for the ship: No point of them in-game, so obviously for the Suicide Mission.
Team Leader: Obviously someone with experience such as Miranda or Garrus.
Tech Expert: Tali and Legion seemed the only logical choices (pre-Kasumi DLC).
Diversion Team Leader: Same as Fire Team.
Biotic: Natural Biotic Asari or "Most powerful human biotic ever", pretty standard.
Admittedly I didn't send anyone to escort the crew back first time, but that's because I thought it may be necessary to not do so in order to save the whole squad.
The only part that has ever got me was in later play through's when I didn't know that it mattered who you left behind to fight when you went off to fight the final battle, so I have lost Mordin once before because I used Grunt to escort the crew, so had less "tough" people holding the line.
Although this would explain why:

Legion died for me. I reckon it was due to Miranda not being loyal anymore. She was loyal up til the moment when she had a fight with Jack, and I sided with Jack. Then there was no going back to Miranda's loyalty, or anything else with her, no matter how hard I tried (as a paragon).

But then on another play through, I just had to see how badly could FUBAR things, so I purposely didn't do any upgrades to the ship (actually I think screwed up and did 1 or 2 upgrades just out of habit but decided to say "screw it" and continued on), nor did I do any loyalty missions. It was spectacularly bad, and everyone but Shepard died on the final mission.

I didn't lose anybody, not even Kelly. I didn't know 'how' to get the No One Left Behind achievement, but I did get it on my first playthrough. I killed some people accidentally on my second playthrough though.

I lost Grunt and Miranda (Grunt was loyal, but I picked him as fireteam leader). I used Grunt as 1st fireteam leader, Garrus as 2nd fireteam leader, Legion as vent runner, Samara as biotic, and sent Garrus with the crew (I was worried that you needed an actual combat specialist to escort the crew). I brought Mordin and Tali to the final boss (because they're some of the squishiest characters, so I wanted to keep an eye on them).
I hated Miranda anyways, however, I was very upset about losing Grunt.

I want to think Zaeed, but I don't think I had that DLC at the time. (Cerberus Network was down at launch, F that, I want to play the game anyway)
and the entire crew (thought they could handle themselves, whimps)

I only lost legion after sending it through the shaft at the beginning, even though it was loyal... I was so upset...

I managed to get my squad to the end without casualties on my first playthrough. Kasumi was my tech person. Miranda was my team leader #1 and Jacob team leader #2. Mordin served as escort and Samara was my biotic. Jack and Garrus took on the final boss with me.
My crew though... they got milkshaked. So now I have a sad Doctor Chakwas and an empty Normandy. I was so bummed.
I managed to get my squad to the end without casualties on my first playthrough. Kasumi was my tech person. Miranda was my team leader #1 and Jacob team leader #2. Mordin served as escort and Samara was my biotic. Jack and Garrus took on the final boss with me.

Legion. I used Samara as the first squad leader and he was my tech specialist.
