While I was looking for some information about Mass Effect 2, I ended up reading about a possible suicide mission in the ending of the game. Based on my experience of Mass Effect 1, I supposed that someone on my team HAD to die. That being said, when I had to pick up a tech specialist in the first part of the last mission, I chose Thane, because even that he isn't a tech specialist, he was the character that made more sense to sacrifice himself for the team.
On the fire team part, I wanted to choose Miranda because she seemed the best option, but I felt that I needed her powers in case I found out any enemy with shields, so I chose Samara as a leader. As Samara is quite old and had a lot of experience in killing, I thought that she was a good choice, but I didn't think that she worked as a "lone wolf" instead of a team leader.
On the Escort part, I thought that Mordin was the obvious choice. When one of my team members removed Kelly from the "Collectors' cocoon", it looked like she was partially hurt, so I figured that Mordin could escort the crew and start helping right away if they were injuried.
My biotic specialist was Jacob. Even that he was one of my least favorite characters, I still thought that he seemed willing enough to help the team when he was the first (and only?) volunteer to sacrifice himself in the beggining of the mission. I wanted him to have a big role.
As for my team, I chose Miranda and Garrus. I romanced Liara, but Miranda's skills really catered my needs to fight enemies with shields, even that I faced none. I felt that Garrus was kinda like my best friend and I still can not forget when he "thanked me for everything" at the end of Mass Effect 1.
So, Thane, Samara and Garrus ended up dying. I just checked a guide and I still don't know why Garrus died. Of all the choices that I made, I think that Thane was the worst, as I wanted to send Tali in the tech part, but I didn't want to see her dying. I think that I will end up reloading the game and try to save everybody.
EDIT: I just found out that Garrus died because I chose Jacob, haha. Well, I'm going with Samara then.

Mordin died on my second playthough, he died about 3 or so times, I kept reloading an earlier save from inside the Collectors base and he kept dieing and I could not understand why since he ( and everyone else ) was loyal. It wasn't untill I did some research on this website that I realised why he kept dieing on me, I had to break up my normal squad of Shep, Garrus and Miranda, I left Garrus on the sidelines to Guard the door and took Mordin and Mirada to thrash the last boss. For future refreence I now know I am supposed to send Mordin back with the Normandy crew.

I guess it's supposed to be hard to kill Miranda? I've somehow managed to do it in three separate playthroughs, and not on purpose. Probably because I never have enough Paragon/Renegade points and always side with Jack.

Because I didnt want to risk Legion or Tali. Although Legion ended up dying at the final boss due to not being loyal anyway....garrus and ashley fool wrote...

Oooookey... man, you really are a bad leader or you do not talk to your team at all. Miranda and Garrus are only two people who actually have experience leading teams so they are obvious choices to be fire team leaders both times. Samara and Jack have shown on multiple ocasions that they are the strongest biotics, so they are the choice for biotic expert. As For tech expert, you have Tali who fixes everything without a problem, and Legion, a computer that can hack anything with his programming speed.
If you do not have anyone with failed loyalty you shouldn't lose anyone like that. I myself had no one left behind from first try, with Garrus as fire team leader both times, Tali tech expert, Grunt taking the crew back and Samara the Bio expert. TBH I'd

Second time - Legion. Only difference was that I did it renegade. All loyalties intact. All upgrades on. But somehow, Legion took a rocket in the flashlight. I keep thinking I missed something, but I can't figure out what.
