Thane ... Master assassin, great at infiltration, can adopt to almost every situation it said... it was lies! he died when closing the door... Mordin at the fire team .. tought he could command a small team as he was a STG member and very smart/tactical .. but nope..

Everyone lived on my first try - pays to be a slightly obsessive completest I suppose, plus the game gives some pretty heavy hints on good choices for various tasks. Mind when Garrus got shot I did swear at the laptop for a moment thinking I'd have to start again - Bioware are evil

And although she's been fighting for hundreds of years, Samara's not really a people person. So she took one in the gut.
Also, panicking about being prepared cost me everyone except Chakwas. I'm going to miss you, Gabby and Ken.
While my buddies have almost all gotten the "save everyone" achievement on their first run, I remember being relieved I only lost two. Especially since my canon run through all three games is a one save, no reloads affair. Garrus making it out was like, priority effin' one.

I got the no one left behind achievement first go, I couldn't understand how people couldn't get it when they were trying to. After that, in my other playthroughs, I just killed off all the boring chracters for dramatic effect. Jacob and Miranda die all the time.

On my first (and ME3 transfer) file I lost 3 people.
Zaeed died while leading the main fireteam (not loyal).
Jack was killed by the seekers (Miranda was my biotic shield).
Samara died while escorting the crew back to the Normandy (not loyal).

Zaheed, Jacob bought it in the suicide mission plus half of the Normandy crew. ( rocket to the face, shot in the stomach, ground-up in to Soilent green.) IT'S PEOPLE!!

Mordin during the Hold the Line battle. And he died because of a bug in the game as best I can tell.
I had everyone's loyalty, I had bought all ship upgrades, all prototypes, and maxed out paragon meter. Chose Tali for first tech specialist, Garrus for first fire team leader. Chose Samara as second specialist and Garrus as second fire team leader.
I had met all requirements, but there was a bug. I didn't talk to Garrus about weapon upgrades. I had, in fact, purchased the Thanix cannon upgrade a ways back in that game. But the game apparently allows someone to die if you don't ask Garrus specifically about upgrades regardless of whether or not you have purchased said upgrade. In any case, I reloaded my save game before going through Omega 4, talked to Garrus about upgrades, and then went. I did choose Mordin to escort the crew back the second time (chose Zaeed the first time) and everyone survived. That's when I got the No One Left Behind achievement.

My first playthrough nobody died. Believe it or not. but my second playthrough Mordin died.. Well, until I re-loaded my save. I can't imagine playing ME3 without any of the squad.

I lost Mordin on my 1st play through, later I found out he was best off sent back with the crew, the 1st time I had sent Grunt with the crew figuring if they came across any problems, Grunt being my toughest squaddie would give them the best survival chances.

I played like a noob the first time.
Tali, Jack and Thane have died on the ship, Mordin and I do not know who at collectors' s base.
But I like this first playthrough, because I knew nothing, all my decisions were made on instinct. This will be my first import for M3.

I only has Miranda die on my first playthrough at the hold the line part, after loseing her loyalty by sideing with Jack. I think I took Grunt and Garrus with me, and I didin't have Zaeed downloaded, thats why she died. (No spoilers, and no guides.)

I admit that i rushed the game the first time XDD.
I never thought that the ship does not improve necessarily lead to consequences. M2 It's one of my first rpg, so,... i'm not accustomed with with the possibility of different end.
I will not say the surprise was mine when a friend told me he had all his commando alive.
I thought, what the hell, what is this game !!

I sided with jack the first time around. Jack was in the right about the arguement, she had every right to be mad. I like miranda but sometimes she is mean just to be mean.
I lost miranda and zaeed my first playthrough because they were not loyal.
