I'm planning to start an insanity run.  To get the achievement, do I have to start a new character, or can I import a character I beat the game with on another difficulty and change the difficulty before entering combat? I have a character I just cleared Hardcore with, has anyone gotten the Insanity achievement this way?

You're fine to do it with an imported character, just make sure you put it on Insanity difficulty before leaving Eden Prime.
