Shepard is not a model, he/she is very likely to have scars because hell he/she has been through a lot of battles
I agree that it is very unlikely that there will be any advances in cosmetic surgery in the next 200 years (after all there have been hardly any significant advances since 1800).

I agree too (except for the renegade=pragmatic part; there are more than enough passionate renegade choices...but that's no excuse for Darth Shepard)

The scars are so horrific that it sort of breaks the whole "romance" thing. What woman is going to be okay with a Shepard that has these nasty, infected-looking scars running all over his/her face? I mean, the implants are causing the unhealed scars to fester, so I imagine there's the issue of pus and the smell that generally goes with infection.

Perhaps the scars will serve as a visible indication of Shepard being half-organic, half-machine - the uniter of organic beings and the machine beings?
...Now that I think about it, perhaps the renegade ending to the trilogy will involve a deal with the Reapers (the scarry Shepard would fit in perfectly), while the paragon ending will be much more on the side of organic beings?
