I managed an unusual death last night.  I was opening the chests in the ruined temple that have ash wraiths in them.  Wynne had a tactic to cast glyph of paralysis if attacked.  She went up a level after the second chest and added glyph of repulsion, so I switched her tactic to use that spell instead.  I opened the third chest, the one with two yellow wraiths, and she immediately cast the glyph.  But the glyph from the previous fight had not ended, so everyone got hit by paralysis explosion!  Except for the yellow wraith furthest away, that is, who started shredding my pc.  Then the other yellow wraith shrugged off the paralysis quicker than anyone in my party and soon after, it was dead me.  ‘Twas the strangest way I’ve ever gotten the Purple Skull.
 
Has anyone else managed to get themselves killed by a series of unfortunate events?

Not the Warden, but I do sometimes forget that the party will attempt to follow my orders exactly.

For instance, I ask both Mages to toss Fireballs in tandem at a specific location. Now one is already in posistion and fires, but the other was slightly blocked, and she goes running up to the near corner to fire hers, and both detonate while she basks in the warm glow of the AoE. Reload....
