Well, being a roleplayer-wise guy (or trying, at least), I'm thinking of redoing my original playthough of the mission, with the same success and mistakes I made, in order to import it into ME3. And that makes me wonder...did it went good for you and saved everyone? Or just horribly wrong and was a insectoid massacre?
This is how it went to me:
- Everyone recruited and loyal except Miranda (lost her loyalty siding with Jack).
- Did one mission or two before going to the Omega-4 Relay, so crew at 50%.
- All ship upgrades.
- Tech specialist: Tali. // Fire team leader 1: Garrus.
- Biotic specialist: Samara. // Fire team leader 2: Garrus.
- Crew escort: That's what it p***d me off the most...I though at this point that I had to go back with the crew MYSELF, and after *BARELY* being able to survive though the vent opening part, I was like "WTF? DO I HAVE TO GO BACK AND DO THAT AGAIN?". So I sent the crew back with no escort. "If they die, it's their problem, but I'm not going though that AGAIN", I said to myself....
If I knew that you wouldn't have to go back, and that you could have sent someone to escort them, I'd have chosen Grunt or Zaeed.
- Taken for the final battle: I don't remember exactly, but an unloyal Miranda managed to survive holding the line, so I think I took a weakling with me (surely Tali), and a heavyweigth (Garrus or Grunt)
The results of my first playthough: Everybody survived, but the crew. Although if I had chosen to escort the crew (which I'd have choose if I knew), the result would've been surely 50% crew saved, and Miranda dead (at worst).

-Definitely not all ship upgrades.. so I think Jack and Thane died in the ship

I like it that he is "pure"... he is suprised by everything and well.. that is what makes him so special for me.. so i wouldn't go back and do anything to him..

Yeah I remember. Legion died because I sided with Tali during their argument and then sent him in the tunnel as tech specialist (had no idea of the mechanics behind the SM back then). I was kinds sad because I wanted to save everyone but it made sense because my Shep was very paragon except that he never trusted any geth (too much history, he also blew up the base during Legions LM).

I remember my heart was pounding from the tension whilst my mind blown away from the sheer awesomeness of that final mission. Final stages in games are always great the first time you play through them. 
Anyway, how it went for me was:
- All squadmates recruited and loyal. The conflicts between Miranda/Jack and Legion/Tali worried me at first, but luckily I was able to resolve them peacefully.
- Ship fully upgraded. Though at the time I didn't really think it important, not until I read online and watched the vids of all the bad things that could happen if you didn't upgrade the ship.
- Tech Specialist: Legion. I thought Tali could handle the assignment as well, but I figured Legion would be a better choice since he is a friggin machine.
- Second Team Leader: Garrus. Thought about it for awhile, but how could you go wrong with your ME1 bro?
Given his history and skills, I suspected he would do a good job, and much to my relief, he did okay.
- Biotic Specialist: Samara. She did make the suggestion after all. And when it comes to biotics you can't go wrong with a centuries old Asari.
- Crew Escort: Jacob. I honestly had no clue at the time that any loyal squadmate would suffice, so I picked Jacob on the basis that the crew would trust him and I didn't want to sacrifice my bigger guns.
- Final Party: Miranda and Grunt. At the time Miranda was my LI and seemed fitting. In retrospect thought bringing Grunt was a mistake, as his abilities didn't mesh well with Miranda or my Soldier Shep (somebody with Pull like Samara and Jacob would have been better).
Result: Everyone lived much to my surprise, as I dreaded losing any of the crew as Bioware hinted before the game's release. So I was estatic at seeing the "No One Left Behind" as soon as the base blew up.

I remember this mission. Frickin' awesome.
First leader was Garrus, Tech belonged to Tali.
Diversion leader was Grunt. Biotic went to Jack and Mordin led the crew escort.
Grunt was the only casualty.

Mordin was the only person to die in my first playthrough. I was so confused at the time. The Normandy was fully upgraded, everyone was recruited and loyal, and I had selected the proper squadmates for all the missions leading up to the final fight.
However, I made the mistake of bringing Grunt with me to the final fight. The worst part was when they just cut to a dead Mordin on the way out without explanation. I feel the writers should have included some clue that maybe some specific squadmates should stay behind for the hold-the-line part.

Hehe, my first time through the game I somehow managed to get everything right, and everyone survived the SM. I didn't read the boards at the time so I wasn't even aware that people could die. My second playthrough I was like "WTF!!!!!"

So in my 2nd Suicide Mission.
In my 3rd playthrough I menage to figure out how bring Legion on flotilla, have romance with Tali and save whole abducted crew.

Jack, Legion, and Tali died during the last "epic" battle in the game, crew escort I think was Mordin,
Tech specialist was: Tali, Fire team leader: Zaeed, fire team leader 2: Miranda
and for the final battle I took the guys who I thought were "tough as nails" ie Zaeed and Garrus....
All a bit vague I know, but since my first playthrough to the game I haven't been back with another character, and have been more interested in developing my exisiting character through the DLC's..

One thing for sure was that I wanted to "spark off" a romance with Miranda, so i specifically went through a number of missions again, so as to not "miss" the oppurtunity..lol,
