Another one you should try out is refusing to let Leliana join your party because she is crazy. Later she meets up with you again and if Alastair is in your party you get some humorous "hidden" dialogue in the conversation
Seems this game has a lot of hidden dialogues that aren't obvious to get. Must be why I have completed the entire game once and almost completed it a second time with a completely different style of character but still only have 61% completion

Regardless of this choice, Ferelden's political unrest is settled and the kingdom is ready to face the Darkspawn at Denerim. At this point, the player learns that a Grey Warden must personally slay the Archdemon, giving their life in the process; the player chooses between sacrifice themself, sacrificing Alistair or Loghain, having either the player or Allister/Loghain conceive child with Morrigan that would capture the archdemon's essence, or use the Sword of Forbidden to destroy the monster's spirit. Regardless of the choice, the player's party reaches Denerim and forces a final confrontation with the Archdemon atop the largest tower in the city. The archdemon is slain, causing all of the other Darkspawn to flee.
The date of this quote is around July 2008 however, which means the sword of forbidden, aka easy way out, may be part of the deleted contents.

According to the wiki dates, at the time they added the note about Sword of Forbidden ending, Morrigan's ending was already present. I guess Bioware really just want there to be no easy way out and no easy decision when it comes to the ending.
Let's be honest. If Sword of Forbidden is an available ending, it will most likely be the popular ending and completely overshadows the other, more "difficult" endings.

Morrigan’s role was actually the first conceived of in the game. Though someone else was the first actual character.

That sounds like a logical explanation, some of the things on that wiki page seem a little outdated (Loghain hopes to help Cailin? When? Where? Are we talking about the same Loghain?).
