I think there's a distinction here that you may be overlooking, though -- it's a difference between actively roleplaying the characters, and just overwriting some of the choices made by original creators of the game with something that you feel suit the companions more.
As example, original Zevran is designed as blonde but someone may feel that his character works better with dark hair. Changing character's outfit with one you pick for them is very much like this, too -- just because a character was created in certain form does not mean everyone will see that form as ideal for said character.
There's also a smaller issue here, and that's the fact these other characters are "their own" to very limited degree. They don't pick their own armour and weapons after all -- they remain locked with the initial gear provided to them by the game/designer and they don't do it as conscious choice/result of being "their own character" but simply as limitation of the game and/or convenience for the player. Otherwise imagine the hilarity that'd be your whole party trying to get hands on that one good piece of loot or armour that dropped.
If you're talking moding, well hey that's cool. I have no complaints there. Mod the living hell out of it, I'll cheer you on and then download them. Just so long as the characters look interesting.

In my opinion, a balance of both would be nice. More than two costumes that are just pallet swaps would be nice, make each of them look unique and stand out rather than blend in with Merc #3423. However, I'd lean on the DA:O system more than the ME2 system.

Well that's assuming that the player really is given the choice of "whatever else." In which case, heck yeah. And more please. But pragmatically, the chances of the player actually geting "whatever else" included along with the apples is slim to none.
So what you're suggesting is indeed what I would prefer. However, I don't think we'll get it one way or the other. So I'd prefer individualized as that's a safer bet.

I didn't care for Isabella's personality in DA:O, but I'm excited to see what she will add to DA2. She's got a lot of character even if she isn't terribly moral. I really like her re-design too, it makes a lot more sense for her character. The only thing I don't get is the lack of pants. The rest of her outfit is awesome, but it's like she was in a hurry and forgot to put them on.
