I've gotten it once (which is surprisingly often considering I've only played through DAA twice and barely travelled around with her the first time...) but I'm relatively sure that I got it when I travelled to Amarinthine. Either from Old Stark's Farm (or whatever the place where you ambush the conspirators is called again) or from the Keep, not quite sure any more. I also think it was very soon after I got her, before I did the Blackmarsh as my second main quest.
It's a shame that her quest is so hard to get, I really like her.

May have figured out how to trigger it. For completeness' sake, I'm going to post everything I've done for Awakening. Either they all had an effect on the quest triggering... or they didn't. Or I'm just lucky.
But it wouldn't hurt.
1) Imported a Dwarf Noble save.
2) Recruited Anders and Oghren first chance I got.
3) Completed every quest up to defeating the Withered.
4) Talked to Garavel, Woolsey, and Varel to get information on the 3 main quests.
5) Did a Day in Court and the first part of It Comes From Beneath. Also gave Voldrik 80 sovereigns and veridium ore to Wade.
6) Accepted and completed the quests dealing with the Turnoble Estate and Eileen Bensley.
7) Talked to Mervis right away, accepted both the Merchant's Guild quests and the Chanter's board quests, only completed the one dealing with thugs.
8) Went to the Wending Wood and progressed through it.
9) After I defeated the dual Dragon boss, I went to the exit. I then saved and gave Velanna the shiny Malachite and the Elven trinket. Then I saw the scene and exited the Silverite Mine and went to the Wending Wood again.
I'm fairly certain that #9 is the only one that matters, but I'm also inclined to mention that I hadn't gotten any random encounters in all of this and that I think one random encounter -- the Burning Bridge one -- causes this quest to become hard to trigger, if not outright impossible.
