Edit: As to how she comes back, knowing what she is would be helpful.

As an aside, it's nice to see you again! I haven't seen you posting in a long time.

I say, keep it a mystery (whatever happened to her). More fun that way.

I hope we find out, but honestly I wouldn't be surprised if we don't since Hawke shouldn't have any prior knowledge abotu her presumed demise. That is of course assuming the players warden went that route.

But like others have mentioned, we've only seen DA2 Flemeth as she would appear before it would have been possible for the Warden to have killed her.
And beyond that, we know from Witch Hunt that Flemeth isn't a blood mage, abomination or even human, so she sort of breaks every rule possible apparently. I'm guessing we'll get some hints here and there but they're likely saving any huge revelations for whenever Morrigan comes back into the fold.

Flemeth's own words when you tell her you're going to kill her suggest she's been "killed" numerous times before: "Come then. It's a dance Flemeth knows all too well."
As for discovering her secret, I'd say there will likely be hints but no big reveal.

Morrigan did tell the Warden that she doubts (s)he truly killed Flemeth and that it was likely to merely be a slight inconvenience to her.

That's what I was going to say. Plus we killed her in dragon form, maybe she can't 'die' that way. Morrigan says she bleeds like everyone else, but probably meant it as a human. Maybe you have to keep her from shape shifting first.

A while back Mary Kirby hinted that Flemeth's human form might not even be her sort of "base" form- which Witch Hunt sort of confirms.
Here is the quote:
Mary Kirby wrote...
She's a thousand-year-old abomination, a shapeshifter who, according to legend (and according to the grimoire you give Morrigan) seduces mortal men in order to get herself more daughters. Her daughters are reputed to be able to kill men with fear. She's both a seductress and a hag. She played up the hag aspect for the Warden, "Oh, never mind me, I'm just a crazy old bat who saved your life! Go on your quest and take my daughter with you! I'm sure nothing terrible could come from that..." But just because the Warden only saw her as the hag, doesn't mean that's all she was...
I would advise you that in any dealings with Flemeth, be confident of nothing.

That would be a little.. hard? would it not?
Even the Templars who are quite famous at negating magical use couldn't stop her from shifting.
Perhaps an explanation to her immortality would be that she is an old god who escaped imprisonment?
So she acts like an archdemon, whenever death comes upon her she simply travels into another body.
Hmm.. that sort of theory works well for her, Morrigan needs an old god to battle an old god?
Lots of questions and not so many answers.
Anyway that body she was in is definetly dead (that is if your warden killed her, but I mean who wouldn't?).
The start of the Witch Hunt dlc says something like: "you get the feeling your standing ontop of a grave."
Again that is if you killed her I think. However her soul is not truly dead, never will be.

SPOLIERS AHEAD
Technically one has 2 choices to kill Flemeth or not for Morrigan. Morrigan even said her mother would eventually be back.
The problem is, even if we meet Flemeth alive in Lothering or around there, what part in the timeline does this take place and when exactly does the Warden decide to kil/let her live?

Killing her is going to be a new staple in the DA franchise. You get to kill her in every game. After killing her in DA-5 for the fifth and final time you get a special "Witch Hunter" Acheivement.
My greatest fear. Recycling the same boss over and over again like Nintendo games. How many times does Link need to defeat Ganondorf before he finally dies?

I'm pretty sure it's before the Warden kills/spares Flemeth. I have no proof, but we know the Archdemon is still around at this time as the Blight isn't over yet at the beginning of the game.

And she's not even a final boss and optional. It'd be more like playing the Warden every game and the Archdemon always being the big bad.

Different Link, same Ganondorf actually, from what I've read... there is a timeline for the Zelda series.

For me, that is a valid point. But also a cheap excuse from Ninty.
Same with Bowser, Andross and many others being the same baddies over and over again.
When Andross was revealed to be the final boss of Starfox Adventures, man...I was so pissed off.
