According to wikipedia, we owe it to the Persians and Chinese before the Romans. Persians used fruit, Chinese used milk and rice with snow, though the Arabs were the first to use milk primarily.

1) yes you can not recruit her. I believe act 2 carries out as normal, except she never asks you to help her get the tome since you don't know her.
2) yeah I love isabela for the banter. Its a shame she isn't very useful on nightmare, otherwise I would never let her go.

1. Isabela is indeed optional. If she doesn't join you in act 1, the Arishok still attacks the city and the events still happen. If the Arishok respects Hawke, or if Fenris is in the party, the duel can still happen.
Edit: If you skip Isabela, you miss out on the Ambrosia at Castillon's Landing in Act 3, so you can't make the potion of heroism. THAT'S RIGHT, HATERS. ISABELA MAKES YOU MORE HEROIC.
2. I think her banter with Merrill is most important to her character, because it illustrates so many facets of her personality that aren't apparent through her usual behavior, and only hinted at. Without the Isabela/Merrill relationship, the lovely pirate would lose a great deal of her depth. There would be so much less sense of her sense of vulnerability and feelings that I probably wouldn't have been interested in her at all.
Aveline comes in second, because their relationship really showcases how Isabela behaves in an environment that isn't necessarily sexually charged, but still shows how strong the two of them are, and how their opposing viewpoints don't necessarily preclude them from learning from each other, becoming friends and growing.
Third would probably be with Varric. They play off each other so well, that it's entirely comforting to watch them talk. It's one of the main reasons that Varric is one of my go-to characters when writing scenes with Isabela, she can be free with him in a way she can't with others, because of that sense of comfort. Also, because the friend-fiction banter between Varric, Isabela and Aveline is pure gold.
