I think of this as a bit of an oversimplification. While there are elements of truth to what you say early on, I would hardly call Isabela's conversations with Aveline over the course of the game to simply consist of sexual jokes and deflection to innocuous topics, nor her talks with Anders in Act 3. I also disagree about the twins and lack of cluelessness. The twins' naivete are one of the sticking points in her banters with them. Why wouldn't that continue?

I tried it to get it on one of my characters by loading a save right before the romance scene and playing on from there, but apart from Isabela's outfit switching properly now, everything appears to be the same.

...and I never planned on liking Isabela. In fact, I was determined not to like her. I didn't particularly like her in DAO; how much could she have changed? Sure, she's a pirate, but look at her! She's clearly intended to be fanservice, and where are her pants? Are her pants supposed to be the relic? No wonder she's so desperate to find it! The last thing female gamers need is an overly promiscuous pandering pantsless pirate to more firmly entrench gaming in sexism.
...but then my Hawke was wandering around the streets of Kirkwall with Isabela in her party and we overheard the conversation Isabela has with Aveline where she says she doesn't care what other people think of her, because they don't know her. She knows herself. So first!Hawke and I tried to lighten up. We let ourselves laugh at her jokes. We forgave her for selling us out to the desire demon. We fought the Arishok to keep her around. We took her with us when we fought the Templars.
...now I'm in love. We're talking head-over-heels, created-a-Hawke-just-to-romance-her, writing-motherloving-fanfiction love. Isabela is an AMAZING character. She knows who she is, and she makes no apologies for being that person. She's one of the first (if not only) characters I've encountered in any popular medium who's comfortable with her sexuality in a way that is both realistic and unashamed. If Hawke gives her reason enough, Isabela will trust Hawke more than she's ever trusted anyone. She's just... she's so... *shrugs* I'm in love.
So I'm here, in a Dragon Age forum, to support an amazing character. I plan on sticking around too, because Isabela is worth it just that much. /soapbox
