I did almost all of the missions with Garrus or Liara but somehow it didn't unlock.
How do I get the "majority completed with _____ character"?

check the Mass Effect wiki. You have to do a few major missions and plenty of assignments to unlock the achievements

Google for the assignment checklist, never take the relevant people out of your party.

There are a certain amount of missions that need to be completed for each companion to unlock the achievement.
For Garrus and Liara, they need to be with you when you complete 50 assignments, the highest number required for all the companions.
Ashley, Kaidan and Tali need 45 assignments, and Wrex requires 40.
You have to make sure that they are in your party for all possible main missions and need to complete as many side quests as you can. If you did not do all of the side quests, or did not use them on a majority of the ones you did do, you would have missed out on getting the achievement.
One of the best ways to go about doing this for Liara is to wait to do any of the citadel quests, aside from recruiting Wrex, Garrus and Tali, until Liara is in your party and to make sure to complete those quests with her to add to her total.

Pretty much follow what anyone else already said and check the ME Wiki. But remember to know who you want from the start so you can accomplish alot with that squad mate in particular and ALWAYS use them. When I did my playthrough as an adept for those achievements I initally wanted Ashley and Tali all the way, did one mission, and jumped to Wrex instead of Ashley. I only got the Quarian Ally achievement. Just a tip.

Im wondering is this achievement rewarded only by one playthrough?
Or can I finish some quest with same character and squad with new game.
I done almost all quest just to have Garrus and Liara ally and didnt get any achievement :/

It took me way too long with Wrex, because I didn't realize it needed a majority in one game, but I really wanted that health regeneration. Now, don't get me wrong, Wrex is cool and all, but variety is the spice of life, right?

My thoughts exactly, which is why i rather detest the ally achievements. Despite this, being the completionist that i am, ( at least for the me series ) i just got the soldier and asari achievements on my engineer profile.
Oddly enough, i got ashley's as soon as i finished the last side quest, pinnacle station not withstanding. I was afraid i messed up with liara's, but i received it during my final talk with saren at the end of the game.
I'm pretty sure this means you need to complete so many missions, as well assignments, although this begs the question of just exactly what mission did i complete while talking to saren?

I heard there was a glitch with Liara's Achievement. A friend of mine whose played the game has yet to obtain that achievement and he told me he took Liara everywhere.

Liara is tough because I think you need to avoid completing ANY quest (including the ones at the Citadel) until you get her. This means that the minute you become a Spectre, go to Therum, get her and then never take her out of your party.
