So you make a new post, and you can't see it in the thread. Even after a hard refresh.
Yet the board index shows you as the latest poster, so it must have gone through.
This issue (of new posts not showing up until seconds or minutes later) was evident at one point on the Wizards boards, and I presume it's the same root cause - making some assumptions about the underlying hardware of the site, when you submit a post, it's written into the database on the master server. This is then replicated out to "slave" servers who actually serve the thread pages. When the replication can't keep up, the delay between you posting and the post becoming visible appears.
Assuming this is correct, possible solutions would be to shard the database instead of using replication, or to simply throw more hardware (in the form of a faster line between the servers, in this case) at the problem.
