My first attempt at a playthrough I never went to Harkin, but still found Garrus at the Med Clinic. Of course, I'd already recruited Wrex so maybe that triggered it somehow.

I'm pretty sure Both Wrex and Garrus are...*optional* party members.
I know I know...but I have managed to leave the citadel without either one of them...that save game slipped to the void.
