See from what I am seeing here there are only two options:
1) it is a choice between Carver or Aveline as to who survives. That choice could have weight added to it due to context we are unaware of, such as it requiring you to sacrifice Aveline via blood magic in order to save Carver. Who knows. It would leave you with a party like this after recruiting Isabela:
Mage Hawke/Rogue/Rogue/Warrior/Mage
Warrior/Warrior/Rogue/Rogue/Mage
Rogue/Rogue/Rogue/Warrior/Mage
Basically hurrah for rogues?
2) It is purely about Carver and there is now an option to save him (or not). This would result in you having an extra warrior in the mix to the numbers above, which isn't completely out of the question considering you sometimes get an indundation of companions at one part of the game in a Bioware title.

Interesting, that's how I originally expected Inferno to be (based on its icon, at least)-- a sort of Firestorm. Sounds cool.

At first I thought that was a kind of ridiculous suggestion, but now that I think more about the situation it seems more plausible. If the choice WERE to be between Carver and Aveline, surely they would have to give some reason for not saving your brother. Such as having to resort to blood magic and sacrifice Aveline to do so. Although this would be assuming Bethany has any knowledge of blood magic, or perhaps Flemeth I guess.
Why Aveline? She has (presumably) just lost her husband and is probably grief-stricken. I can see several potential circumstances that could tie in to her willingly sacrificing herself (though it could be unwilling), but with so little information about the plot of the game I can't really assume them.
Also, seeing Carver getting smooshed in that cutscene, I can't really see how you could actively stop that from happening. Especially since you're not supposed to know what's about to happen before you play. Therefore if they were to be any choice involved, it would (imo) be more likely to happen AFTER said smooshing, as Carver lay dying.

So did you get a feel for any of your party members' personalities? Anything on what they are like? How are the VAs, what do the characters like Isabela and Carver sound like?
Carver was not present, no one spoke apart from Isabela and Hawke, so no.
Did you manage to hear Hawke's voice? Did it sound like Vaughan?
Maybe actually... He had an english accent (a proper one) and didn't sound as hostile

It seems to me some choice made before that cutscene could change what would happen. It could be something like encouraging Carver to be brave, or giving him some reason to prove himself or something.
