At the flying platforms segment of Collector Ship, I literally just gave up on getting the insanity achievement because I kept dying.

That's what Locust + Warp ammo is for. Or pick up Sniper Rifle specialization.
Although it's all medium range in that part, so the Locust+Warp Ammo is the best choice. Maybe coupled with sniper rifle + incendiary ammo for the Scions.

ME1: Husk Ship. Stupid EMP-Husks stripping your shields and then bum-rushing you to death.
ME2: Garrus' recruitment mission can be tricky if you attempt it at early levels without any upgrades. Same with Tali's recruitment mission.

Insanity really was not that bad, if you played Infiltrator (well, that's the only class I've played so far on Insanity). Just sit back, let your squad soften everything up first, let them die, and pick everyone else off with your sniper, and if they get up close, use the SMG. Also, be sure to have warp ammo.
*On topic, the hardest one for me was probably Freedom's Progress (on Insanity). I still hate those armored husks...

I loaded that game 10 times on my first play-through with my canon Shepard. My 2nd Shepard had a slightly easier time because of the unlocked abilities and what not. But still... at east one of my squaddies died
ME1: Luna missions
Not exactly difficult but suddenly getting slapped in the face by barrage of rockets from the drones was kinda like "Whooah okay maybe I should not have under-estimated this mission"
ME2: LOTSB
Chasing after Tela Vasir with very little cover & also the fact that she keeps teleporting
ME2: Horizon
Edit I forgot to add this one. Died during the mission both before and at the final part.

ME2 - collector ambush. i have a sentinel stuck there from a few months ago. hell be stuck there forever it seems.

Kasumi's loyalty mission can be a PITA if you do it "first" to get the Locust.

Place once squaddie at the end of the cover to your left. and one squaddie at the apex of the forward part of the initial platform. place yourself at the apex of the immediate cover.
In that configuration your forward squaddie will die from harbingers first assualt probably but you should be able to take harbinger down if you pause to aim.
After that revive your forward squaddie and move the left hand squaddie behind other cover for the when the left platform arrives.
Wins.

If you're using her shadow strike you might wanna try pausing and placing her back in cover again and again and again. Keep her in cover on the right part of the first cover... you need to take up cover on the left side.
Always command her back after shadow strike.... otherwise she will jump forward over the coevr and be mowed down like a girly japanese theif jumping out of cover.

I understand. Some mercs are as hard to kill as if they all had Colossus armors. Ammo mods (chemical and incendiary) are a real must there.
