Whoever you let die on Virmire, does all the stuff they have equipped just disappear? I am replaying through and I gave Ashley some of the best heavy armor around but let her die on that mission. In reality, yeah, that stuff would be gone, but I wouldn't think they would do that to you in a game.
I am pretty certain whatever gear I had her equip is gone now. Am I correct?
So do you lose all the gear when that squadmate dies on Virmire?

Yes.
You must plan ahead, decide BEFORE you go to Virmire and change his/her kit. The one you leave to DIE, DIES along with the equipment that he/she is wearing = blown up by a 20KT Nuclear Bomb = not much left to retrieve.

Yes, Buster is right. Unlike Wrex; if you kill him, you will automatically get his stuff back in the inventory.
