Overlord ending to me was sad but to me it was more disturbing
I still can't fathom why David's eyes are kept open and huge tubes stuffed in his mouth, his brain was to be wired to the geth, half expected to see top of his skull opened up and mass of wires hanging out, but what we are given to see just made me question the mental state of the writer/artist/programer for that scene...didn't make me sad, but did want to stuff a grenade in Archer's throat.

Lots of good ones mentioned so far. Not sure if anyone mentioned Mordin's battles with his conscience during his loyalty mission. Very well written and poingant without bashing someone over the head. Instead, it was nuanced and textured: a man struggling with the aftermath of a difficult and momentous decision.

Tali finding her father dead.
Kasumi's pleas on her loyalty mission.
Visiting SR1 site.
Overlord ending.
And of course Liara, my LI telling me its been 2 years.
Of course, very first playthrough which i did not complete - watching kelly die. (loaded save and fixed that)

The one that always chokes me up is the Quarian mother on the Alarei, just makes me think how pointless the whole Geth-Quarian war is. No one will win in the end. Plus I'm a momma's boy.
Turning down Jack and the renegade conclusion of her loyalty mission feel like the most evil thing you can do in the game, just super depressing.
Shepard getting wasted, then coming back to a sea of apathy. The Alliance has their council seat so they don't care, Liara is bent on getting revenge for a person she knew for maybe a week tops, the VS thinks you're a traitor... depressing. Plus, for being your mentor, Anderson isn't very trusting or supportive at all.
The krogan father on Tuchanka.
Garrus' continuing failures.

I think Garrus' loyalty mission was quite sad if you talk to Sidonis, then choose the "I can't help you" dialogue, to which Sidonis replies "No more sleepless nights".
Also in the Overlord ending I was thinking "What the hell is this?"

Tali's loyalty missions.
It starts with the fear in Tali's voice, continues with the video of the Quarian mothers dying words, and finishes with the discovery of Tali's father's corpse...
That whole mission was just one long string of sad.
