Don't know if this has been discussed, the search function on this forum doesn't work, so apologies in advance if it has.

I was in Redcliffe castle, and I chose to allow Jowan to kill the mother so I could enter the Fade, and then I picked Blood Magic from the demon. Once the cutscenes were over, I reloaded my saved game from before I had Jowan kill the mother. I found that I still have Blood Magic unlocked, and my quest still said that the mother sacrificed herself, etc.
However, when I did it again, this time with the Circle Mages, and I killed the demon instead, my quest STILL said the mother was dead - even though she was standing in front of me.

So I was able to save the mother, unlock Blood Magic, AND kill the demon.

It's an exploit, yeah Unlocks are tied to account and not saves!

Achievements are tied to account, not save game. It would be annoying frankly if they'd be tied to the saves. So once you've unlocked these achievements, they stay unlocked for your account. No "bug" or "exploit" here, sorry to burst the bubble.
