I'm pretty sure the circle of magi must, since they're the ones that prepare the archdemon blood for the joining.  But do any other organizations know?  I'm mostly curious if there are any indications that the chantry or the templars know about it.

Even if it is known, it's more likely that only people of high ranks in those factions know. It's not the sort of stuff everyday mage apprentices chat about.

Just because they prepare something doesn't mean they know what it is for. Also the final ingredient is not given to them, and is mixed by the Gray Wardens.

That was my impression when I did Ostagar, since the mage already finished their preparation when you returned with the blood. I think what they make is the catalyst and it was then mixed with the blood in the goblet to make the final drough.

The reason they want to keep it secret is because a lot less would join if they knew they could die from trying. Apart from that there's no damage in people knowing how it's done. If they drank blood by themselves, they would either die or become immune, and the latter would be a good thing apart from the approximate 30 year later death sentence.
