Insanity achievement not awarded upon completion

Hello.
I recently finished my first playthrough of Mass Effect 2 with an imported character from Mass Effect 1. When I finished the game, I selected to go back into the game and I checked my achievements. Everything that should have been awarded upon completion was, except for the insanity achievement. I selected the insanity difficulty from the start of the game and never changed it during my playthrough. I even checked the settings several times throughout the game to make sure the difficulty wasn't changed, and it was still on insanity.

I checked the forums, and it seems like this is a problem that has happened to some other people as well. I made sure my game was registered and played through the end again this morning (and went back into main menu after credits), but still no achievement.

Does anyone know how to fix this problem? I have Mass Effect 2 1.02 with the following DLC installed:
Shadow Broker
Kasumi - Stolen Memory
Overlord Pack
Zaeed - The Price of Revenge
Cerberus Weapon and Armor
Normandy Crash Site
Cerberus Arc Projector
Firewalker Pack

I also have a Intel Core 2 Quad 2.40Ghz CPU, with 2 GB of RAM running on Win XP Pro SP3, and an Nvidia GeForce 9800 GTX+ card.

Dont worry about it. Its not like the acheivements mean anything, unlike ME1 where it gave actual bonuses.

Agreed. I just spent near 60 hours doing every mission (except overlord and LotSB) on insanity, and I did not get the achievement. I set it on insanity initially, and didn't touch it at all. I am VERY annoyed!

same here. any solution to this? finished the game four times, once on insanity. no award :(

Has a workaround been offered by Bioware yet?  I had the same experience as the original poster.  My guess is that by checking to see if the difficulty setting has changed, the achievement gets borked.  What I mean is, by simply entering the difficulty section of the menu, the achievement fails; even if the actual difficulty setting was not altered.  This is a guess though, but the only possible scenario that makes any sense.

I have no issue trying for it again, but I would atleast like to know what I need to do as a workaround for this bug.  Is it a menu issue?  Is it related to DLC content being integrated into the game?  (I mean, the extra upgrades and powerful weapon and armor options do make the game a bit easier on insanity mode)  Or is it a total enigma?

Try looking at the save with this editor just before you finish the game........

IIRC under plot tab, there is a box to check or in your case uncheck for changed difficutly, check and see if it is checked and if it is uncheck it and relplay the end on insanity.

Cool, thanks for this tool. It shows clearly that the difficulty had been changed, even though I only looked in the difficulty part of the menu a few times to confirm that it had not. I'll run some tests and see if looking is what causes the problem. Then I'll report back here.

Well through a series of tests, it seems that looking in the difficulty part of the menu did not cause the problem; nor does using DLC equipment. The only things left to investigate is hitting resume from a death screen and more importantly having upgrades greater than what the original game intended before DLC content. I believe 5/5 is the max amount intended for launch.

Chances are the glitch is just a fluke, and nothing that can be recreated with testing, except as a random occurrence. In any event, thanks again for the tool.

Just curious: are you folks playing Insanity difficulty after several playthroughs of the game? I'm on my second playthrough (and I suck, I guess), and I keep dying...too frequently, and it's on Normal. I saw a new "Bonus" power listed called Reave, so I picked it when I started the 2nd playthrough. Someone suggested I should have gone with Barrier instead. Is it going to make a really huge difference?

Is there a way to start another playthrough so I can test it? Is my only option to just load a late-game save and finish the game again and let it start over again? Is this going to muck up my current (2nd) playthrough? It's hard enough as it is, and I don't have a whole lot of time to devote to gaming, and it tends to come in huge chunks, then gone again. Thanks for any help.

If your second play-through is with the same character you beat the game with the first time (now at level 30 from the beginning) then you'll have a tough time. The enemies scale to your level, but you won't have any upgrades yet.

If your second play-through is with a new character, then you will have access to all the bonus powers that your companions unlocked when they became loyal. For insanity, I recommend Geth Shield Boost or Barrier since they can be upgraded and Fortification cannot.

The nice thing is, that eventually on the Normandy you'll be able to re-spec not only your bonus power, but all your power distribution points eventually. I'd just keep playing your character until you get to that point in the game, then re-spec as needed. If your playing a level 30 character from the beginning however, then I'd start over.

On a side note, Reave is pretty powerful,
