I'd be fine with something as simple as an online co-op version of the regular story. No bells and whistles, just Player 1 is their Shepard and Players 2 & 3 are squadmates of their choosing.
This +1
Edit: I'd like to add that I think coop would be great because the squad ai sucked big time. Especially in hardcore difficulty. I was constantly left on my own trying to kill off a pretorian or 10 baddies and reviving my teammates waste of the waste of medigel when they dies 2 seconds later.

What if there was a multiplayer co-op campaign where you could play with your friends as people on Earth and you weren't shepard and the host player did the diologue options as their main hero
...No. It's this sort of thing people are terrified of, and frankly I don't blame them. Something that complex would take away development resources.
If there is multiplayer, it should be co-op, yes. But, if there IS co-op, it has to be something super basic like what I described. Two players man the squaddies, while Player 1 controls Shepard.
Do you really want a million threads about how some jokers didn't like the game, and therefore it was multiplayer's fault? My method is the only way to prevent these boards from becoming more inane and stupid than they already are.

Ultimately co-op would destroy the whole purpose of Mass Effect; making choices that have far-reaching consequences. Then what happens after you jump back into single player? Do other players' choices now affect your game? Of course BioWare could allow both players to make different choices during cutscenes but this introduces another problem. How can BioWare possibly create radically divergent storylines while ensuring that co-op players be able to experience the same events?
For me co-op is out of the question. Everybody wants to be Shep and that simply isn't possible. Now what I could see is an Alliance vs _____ scenario. One intriguing "mode" that comes to mind is some sort of defend the "fort" variant; the "fort" being an alliance vessel.. Team 1 would take the role alliance marines repelling team 2 (an enemy boarding party consisting of Husks/Collectors/Geth/Cerberus/Pirates etc.).
Could make for some fun online.
The only way I see co-op actually working is if they create an entirely separate co-op campaign ala Splinter cell: Conviction. They could write the story in such a way as to allow both players to make choices and work as a team. This, of course, could potentially take resources away from the single player campaign which I'm sure many of us wouldn't want to see happen.

Nay. For most people however, co-op is a happy medium but I'll tell you why it won't work. Who wants to be some thug tagging along for the ride while your co-op buddy plays as Shep? My guess? No one.
I'd love to play as Grunt, actually. I already enjoy watching other people play the game. Playing as a squaddie just allows me to get involved in the combat when there's no dialogue going on.
You want to be the guy making all the cool story decisions m'right? Furthermore, who gets to play Shep? Is the decision made randomly? Do we paper, scissor, rock for it?
I can play through singleplayer if I want to make the decisions, or my friends and I can decide "all right we'll do Schnei's Shep this week." The host plays as Shep. Simple.
