Jacob because he's pretty one dimensional. Plus he called Garrus a cuttlebone

Jack - Mentally unstable, unable to keep her immense power in check, and openly threatens my crew and myself. Too much of a liability to keep in the long run.
Thane - While I don't hate the guy, his sickness obviously deems him a likely candidate for another "suicide" mission.
Jacob - Like Thane, I'm not killing Jacob because I hate him, but because of his nasty habit of volunterring himself for suicide missions. Hey, he asked for it.

Zaeed: Badass, but it's TIM's money, not mine. He's such a tough bastard that I'd expect him to survive it somehow, though.
Jacob: A nice guy (who'd I feel guilty about nuking), but expendable. And, honestly, pretty much useless.
Thane: He's dying anyway. I helped him atone for his sins, and an honorable death would be doing him a favor.
A close runner-up is Jack, who, even though I like her, is incredibly dangerous and risky to keep around. Miranda only barely missed the list, but that's because she's damn useful in combat. However, I'm one of those bleeding-heart wussies that saves everyone in most of my playthroughs, anyway.
Too bad ME1 squaddies weren't on that list, cuz Kaishley Willenko would totally bite it. *ducks and runs away*

The three I let die in the playthrough that'll go to ME3
Jacob - hey, he volunteered to go through the pipes, not Shepard's fault (plus Ash is still alive, so once her and Shep make up, he'll still have an armory officer)
Jack - way too much of a wild card, not to mention a stone cold murderer by her own admission, and it seems like the only way to get her to actually change for the better is to romance her. My Shep didn't, so Jack became seeker swarm fodder
Zaeed - As badass as he is, I like the idea that he dies saving others for once, plus his last words were that he expected it to go something like this... he gets to go out in a blaze of glory (though I wish it could've been more than a stomach wound that took him down)

Tali - rocket headshot every single time in that clanky bolt head of hers. Die annoying quarian gipsy. Thank you Bioware. I just wish there was some blood and gore upon rocket entry in her face haha.

Garrus - whether male or female Garrus is my protege and trusted friend/lover. I love the kid and I love how he becomes a perfect renegade companion for a renegade Shepard. My favorite.
Thane - again whether male or female he is a good friend of mine. Reliable, trusted, experienced. A pity he didn't have a role in the SM. Hopefully they find him cure.
Jack - best biotic, crazy and LI for my male Shepard. Very similar upbringing to him.
This goes to ME1 but I couldn't help but like Kaidan (he reminded me (what are the odds) of Carth from KotoR which is by far my favorite game.). So there goes number 3 on my list. He was LI for my female character too, best budds with my male character.

Jack, Miranda and Jacob. I find Jack rude and annoying; Miranda is...well...ask Wilson, an ice queen; and Jacob is fairly dull and obessed with priiiiiiize.
Not big on Samara either but she will probably leave the squad to do her justicar stuff.

I actually would like Jack more if she had a more developed "friendship" path. She pretty much just shuts down if you don't pursue her romantically... as does Miranda and others, but I felt it most with Jack and her 'tude. If my Shepard could have been a closer friend with her, I might change my choice. (Thane is borderline because he has "accepted" his death and has made his peace. So, he might substitute for Jack.) Never really liked Jacob... mainly because having femShep talk to him was like watching an example in a sexual harassment video. It was just wrong. Would have to be:
1. Kasumi: seemed like she should stick to her day job
2. Morinth: soul sucking. Nuff said
3: Zaeed: may be an excellent fighter, but other than money nothing is keeping him there. Also I'm pretty sure he thinks he's better than Shepard and is just letting him think he's in control. Not to be trusted

i go a little higher but not too high:
* - Jack (occulus laser, for some reason this one only works for me if you romance her before going in)
* - Tali (engine room disaster, really works with joker's reaction, with what he said after you recruit her)
- (no one dies during the third attack, because the laser cannon is a cooler and longer scene, as an alternative to the other two where the living scene is worse)
* - Jacob (closing the door, rocket, blood everywhere)
* - Grunt (locusts, kind of works with the idea that he would accidentally lag behind during the fighting)
* - Zaeed (second fire team leader, cool line, given his history)
- (no one dies during escort, but i send Kasumi anyways, loyal....i guess on my playthroughs kelly always ends up dieing, adds abit of dramatic tension to the scene...)
- (i take Thane and Miranda to the final boss, since they're the box-art team...they're cool. i think picking thane first makes it so miranda is the one you catch at the end, also those two get a good series of responses if i elect the paragon decision about the base, both survive, i later go on to romance miranda in post-game dlc content
)
- Ground Team: Mordin +1, Morinth +2, Legion +2, Garrus +4....ensures the ground team lives. (i could go either way on samara or morinth but i think the morinth path is more interesting for alot of reasons....)
But yeah, those 7 seem like a decent group to take into the next game. Stealth, Tech, Sentinel, Sniper, Biotics, Medic...thane doesn't have really anything unique but it really seems like they're going to do something with him reguarding his impending doom and the drell homeworld so yeah bringin him. Tough choice on Tali but it sort of works the way it is now, she probably won't have alot of direct impact on the quarians homeworld thing...plus i don't think her romance scene is that interesting...Mordin and Kasumi's loyalty missions leave plot threads that seem to demand they live to continue the story on in 3. Kasumi will probably have alot of interesting things to say reguarding the black box if you elect to keep it (reapers attacking earth, the alliance knew....), the rest of them all seem to have some lingering threads to be resolved. Also the powerset between those 7 remaining seems to be the most diverse while still maintaining the maximum amount of dramatic scene splicing into the finale. (Also it's worth noting that this decision is with default male shepard.)
