I let Valania die in my third play through. Just to see what the outcome would be in the end. I am such a MONSTER
Its actually a bit sweet. Fenris says "I would have given you everything" before Kali Maing her.
Maaaan now I have to go do this on a reload or something, damn you guys. I couldn't bring myself to do it before but I want to hear this.

Well...you had me up until Fenris somehow became Michael Jackson...>.>; No sexy elf should become a white woman. Just no.

Varric, since I let his brother live, even told him in a very sweet way something like "Elf...Fenris, believe me this is the last thing you want to do" I love how he used his name this time.
And during Gamlen's quest, Fenris told Charade something about the importance of family, supporting my advice to go and meet Gamlen.
He might not see his sister ever again, but I think he is happier this way. Yes, he knows now that he competed for the markings, but he is not a slave anymore, I think he will come to terms with his past and build a future with my Hawke

I'm talking bout black Jackson, not that white stick they replaced him with.
Now I'm picturing Fenris with a poofy white afro...

I think the cruelest things you can do to Fenris are A. let him kill his sister, or B. set him adrift again. I really think that's meaner and more insidious than just outright killing him or handing him over.
I was moved at Hawke's plea for Valania if the sibling died in the Deep Roads. She says "I killed my brother, I know what I'm talking about."
This time through I'm letting Carver die. I'm so going to let Valania live just for that.
I always approached it as it's not Hawke's decision to make... it's up to Fenris and what he feels like.
Either way he feels alone... letting her live didn't seem to help

The only time my Hawke prefers to step in is if the game forces me to choose, or a bad decision will endanger others. (Like, my Hawke kept the tool away from Merrill.)

Honestly, I let his sister die because I believe my companions should make their own decisions in times like that. It's their life. If they asked an opinion I would give it but I wouldn't be handing out orders on stuff like that. My Hawke basically put up his hands and was like 'Hey, it's your family, your decision'.

Well, letting him kill his sister is endangering his sister.....so there is that.

What she says to him if she lives just seems even worse than her dying.

I agree with this at some points, but the way the dialogue plays out you're not ordering him not to, you're talking him down from it.
I think sometimes it's better to try and stop your companions from doing something they'll regret, because sometimes they're just not in the right frame of mind. It's not exactly Fenris' most rational moment.

I've let him handle it in two of my (almost) three playthroughs.

A small list of characters that have events where they need saved from themselves:
Merrill-Mirror Image
Fenris-Alone
Varric-Haunted
Anders-Dissent
Avelline-The Long Road
Fenris states on several occasions that the hate that he's filled with bothers him. He doesn't like it and wants to move past it. I think killing Varania just compounds that hate, which is why I usually talk him into letting her go.

That would be pretty darn awesome! I would love to see Varric and Fenris especially doing the Thriller dance. I loved that mod from DA:O. It made me wish I had a PC I could play it on. Alas, I'm just a lowly console plebian.
I wouldn't mind a DLC with an ending like this: www.youtube.com/watch
Hehehehe! I'ms such a sap! That movie never fails to cheer me up! Such a lovely character!!!

Act 3, Gamlen's Greatest treasure. Triggered by visiting Gamlen's house at the beginning of act 3 and reading a 'crumpled note' on the bench near him.
