In this, your character (and your family, if you chose to have one) would have, in addition to a health bar, a hunger bar. For this, you would need to eat food, and it would increase the hunger bar, which decreases over time. This of course means that for foods you would have to implement a new "hunger" property. Stronger things, like Horker stew or something, would have high values. Also, you could implement a health/sickness system, where doing something like eating raw Horker (or many raw meats) would have a chance to make you sick. And alcohol consumed with lower hunger would have higher chances to make you sick.
And you would need to sleep. There would be an optimum time, say twelve or fourteen hours of normal play, but then your character would start to get sleepy--by lowered skills, maybe blurry vision, and if it gets bad enough they just collapse and fall asleep. The optimum sleep amount would be eight hours, which provides the fourteen hours of wakefulness. Less sleep would have some, to increasingly drastic, affect on your wakefulness, and more sleep would improve it by a small amount.
Couple of mods out there that you will love to bits I wager. Number of mods do just this and then some. personally I'm really enjoying combination of basic needs mod (eat,drink,sleep,get ill from stale food, get drunk, alcohol poisoning..) and a survival mod named Frostfall. Latter brings cold weather to the mix. You need to dress warm and/or seek shelter when it gets cold enough. Also enables you to build tents, campsites, campfires. When it comes to Nexus bashing on last page..let's not forget trees for the forest! Visiting Nexus is much like taking a subway ride. Sure, you have that creepy old funny smelling dude in anime shirt touching himself as he looks at you..but then on the other hand, it is a subway! It takes you to pretty good places. Nexus provides a pretty awesme mod managment tool and dozens..if not hundreds of utterly awesome mods that have little or nothing to do with ******!

Just that you need to get your hands dirty. Very dirty.
