Insanity, because it is the most fun for me. I am also used to it, if I changed it to Hardcore or lower, some of the mechanics that I use all the time would be redundant and game too easy = no fun for me.

For months I played on hardcore and was really wondering how anyone ever played on insanity, but after watching the videos posted here, and using the tips in these threads I finally played on insanity and its quite a challenge. Now I only play on insanity. The great thing is that the strategy debated here will really help on lower levels so I appreciate the folks that first played thru on insanity and posted the videos.
btw if you get sloppy you die in a hurry, even if you've played a lot.

Insanity because its no fun when you can blast up everything in a few shots and i like it when enemies try to flank you and not stand and watch like they are stupid.

Both Mass Effect 1 & 2 = Insanity
Mass Effect 3 when i buy it - immediately Insanity, no lower diff levels

Casual, as well. Not because I suck, but because I play Mass Effect for the story and only for the story. I've never really seen it as a challenge; I can play shooters for that.

Hardcore. I've beaten the game on insanity (ME1 and 2 are the only 2 games I've gotten all achievements for), but I prefer an experience that's slightly more relaxing.

ME1, Veteran in my current Infiltrator run. Combat there is not great. A higher difficulty is not worth it.

I prefer normal. I do insanity runs just for the achievements and then go back to normal. I like to play for the story, not to be frustrated over dying right at the end of a segment and have to repeat the last 7 minutes. I didn't even plan to try for the insanity achievement on ME2 but my bf gave me a hard time and said I couldn't call myself a true ME fan if I didn't do it on insanity so I did just to shut him up. And to this day HE still hasn't been able to finish it on insanity. Me-1 Bf-0

Usually veteran. There's no need for the higher difficulties for me. Except to get the achievements. I might play on higher ones if shields and armor would not block biotics. Them doing it breaks the immersion for me. So it's veteran for me.

ME1 is irrelevant for me. I'm currently blazing through a playthrough, and I started on Casual and then bumped it up to Hardcore because the combat was too easy. I was going to just finish it quickly on Casual because I'm trying to get my male Shepard ready for 3, but I figured I should enjoy the ride a bit.
ME2 I started on Veteran, but quickly moved to Insanity once I got used to combat. Can't go back. Even Insanity isn't terribly difficult.

Started on hardcore because I made the assumption that insanity would be just as broken as the previous iteration but made the switch when I first started making videos. Don't see myself ever going back, it's now insanity all day every day.

mostly Veteran, by Legions loyalty mission I play on Hardcore, then by the time I start LotSB I play Insanity.

Veteran. Challenging enough without being frustratingly punishing. Hardcore is used only to get Geth pulse fifle.

Easy/Casual or possibly normal (can't remember if I did the latter or not), but when I get it on PS3 I am going to try to have an 'insanity' playthrough as I'll admit I've become a bit of a 'trophy ho' as of late :/ 
As others have said, likewise I mainly play for the story. Kind of nervous (and yet excited) about doing my first insanity run. Been reading all the excellent guides about doing such with various classes. Still not totally sure which class am going to pick

ME1 I normally played on Hardcore, because, as was stated by others, Insanity was just an annoying time sink. I did it for the achievement, and then went back to Hardcore.
For ME2, I started on Casual so I could experience the story to its fullest. Then I moved to Veteran, then to Hardcore, then finally to Insanity. Now I only play on Insanity because, as has been said by others, anything else feels too easy, and less rewarding.

If i'm just casualy going through the game enjoying the story i play it on Vet, if i'm trying out new combat methods i usally bump it up to insanity to give the method a throuogh testing to see if it can work in the harshest of combat enviroments, but i'll play the game through on hardcore or insanity for specific reasons such as achivments, getting specific weapons such as the geth pulse rifle so on so fourth.

Insanity in ME2, but it is less fun in ME1 so I'll lower the difficulty next time I play.

ME1: Veteran, once through Insanity's Immunity-spamming was enough.
ME2: Insanity right out the gate and hasn't changed yet. I'm actually kind of curious how the game would feel on Normal since that was Bioware's baseline intended experience but I fear I would be horribly bored lowering the difficulty to that now.

I play all games on the highest diffictulty available. ME1/2 was no exception.
