Why is this?
It seems as if there is a delay of some sort because the Leliana's Song Vendetta trophy has shown up after Redeemer, I'm Kind Of A Big Deal and Indestructible even though I finished Leliana's Song several weeks ago.

I let Loghain sacrifice himself 2-4 days ago and I got the Ultimate Sacrifice trophy in-game. I also got the Achievement Redeemer added to my social.bioware profile. A day later, after fiddling with the saves because the trophy didn't show, I got the Perfectionist trophy (found all possible endings) from that same playthrough. But the Ultimate Sacrifice trophy still doesn't show up among my other trophies and achievements in my profile.
Why is this?

There isn't supposed to be a delay aside from the one between unlocking an achievement and somebody triggering the persona update. Unless you unlocked all the Leliana's Song achievements in one session they might have uploaded as two batches (achievements are unordered).
Were you online when you unlocked Ultimate Sacrifice? I'm unsure how achievement uploads are handled and to what extent profiles are synchronized (on Steam, achievements aren't registered if you're not online [edit: when unlocked]), but I'd say it sounds like the web server doesn't register the achievement as unlocked. I'd suggest replaying to unlock it again. If that doesn't work, the game might be blocking the unlock because it thinks the achievement is already unlocked -- I don't know of a way to tell the game otherwise.

I've run into the same type of weird delays and I really couldn't tell you why it happens, but I've had Achievements upload to my profile anywhere from almost right away to up to 3 or 4 days after...
Had about 6 of them that didn't appear and then all uploaded at once a little while back, so it's not just you.

No, that's the delay I was talking about. Achievements have never appeared in real-time. The upload itself does not trigger the feed alert, however; only accessing the persona page does.Deathwurm wrote...

I don't believe I was logged in at the time when I got the Ultimate Sacrifice. I wasn't aware that I could get it with Loghain sacrificing himself. I thought it was strictly main characters who could get it.
If I'm not online when I get a trophy, I usually log in afterwards and go to the trophies section on the PS3 to get it to update. I did that this time too, twice, but it still hasn't shown. I have also returned to that playthrough and saved the game again to see if that helps. All it did was update the persona character details.
I don't believe I can replay the last battle because I have overrun that save. I'd have to use a save much earlier than that. Not keen on doing that seeing how often the game crashed in certain areas.

I am sure i sacrificed Alistair and Loghain in one of my playthroughs *save and play*. However, the achievements have not shown up for those for... quite some time. I did this like a month ago. I know I did this because I recieved the "Perfectionist" achievment.. doesn't that mean that I DID go through both sacrifices?.... hmmm

So it has been a year since I had this problem.
A few days ago I started playing one of my old characters again and got a trophy. I wasn't signed in at the time but signed in the day after as well as checking my Persona page. The new trophy, Sacreligious, has been uploaded... but the Ultimate Sacrifice achievement still hasn't showed up.
This must be a record!
