is it based off of the amount of game-time passed or the amount of plot completed (% completion of the game)?  I have this VERY  uncomfortable stress to do everything as fast as possible and minimize travel time as much as possible and i find myself wondering if its necessary...?

...and does it truly matter?  (no spoilers if you can help it)

As far as I know it just gets larger and larger with no actual effects, so don't worry about it.

Think it's amount of game completed. I've done a LOT of traveling around taking care of side quests. Barely touched any of the main storyline and the blight is still in the wilds. Hell, Lothering is still there!

Completing the Main Quest related quests (those found under the Blight section of your journal) advances the blights spread on your map. As soon as you complete the first (doesn't matter which) quest, some areas will be rendered in accessible.
