As much as I love Garrus he would hate being the XO. He's more a man of action and I would prefer putting him in command of the squad out on the field, if Shepard isn't there, rather than having him handle the administrative stuff.
As for Jacob antagonizing Tali. lmfao. Ok. He said something stupid, but that was that. I think it's one of those "he didn't think before he spoke" - moments and afterwards my Shep went "ok Jacob, foot in mouth."
I don't like Miranda, at all. I've tried to like her, but I just can't. But, if she handles her duty as an XO that's fine by me. As long as she doesn't get in the way of my Shep's work.

Revised opinion: Miranda remains XO, but if Shepard is killed, Garrus is placed in charge of the mission.

Miranda's loyalty with Cerberus needs a good, hard shaking so it can be replaced with loyalty to Shepard. Demoting her would have the opposite effect. H*** hath no fury like a woman scorned, and Miranda Lawson is one h*** of a woman.
Other considerations aside, she has "rank" in Cerberus, has plenty of ego, and can't be removed from the crew and left somewhere. She also happens to be very highly qualified. She feels threatened by Shepard as the "new favorite" and is clearly jealous of his/her fame — and the loyalty in others he/she inspires, something Miranda can't do.
Therefore, my Shepard takes her on all missions where Cerberus clearly committed great sins and really messed things up: Overlord and Jack's loyalty mission to the prison and torture chamber for little kids, in particular. On her loyalty mission my Shep gives every paragon response, determined to save the sister and console her for Niket's death, and so forth.
And Shep brings her back alive from the Suicide Mission.
By the end, I'm confident that her loyalty to TIM isn't as absolute as it once was. I still don't trust her completely, but at least Shepard hasn't made an enemy of her.
As for making her the assistant team leader on the Collector base, my Shep resolves that by picking her and taking Jack on Shepard's team. Jack's the only one with a serious issue with Miranda as second team leader. Even Garrus shakes his head when Jack sounds off in the planning session about not wanting to follow the "cheerleader."
