Sexualized outfits making a comeback (plus all the problems that came with that)
Side lining of current LIs for a new set.
Characters being written by writers who don't quite 'get' the character.
Plot holes.
Paragon equaling "I win".
Making Shepard even more Sue-ish.
What would piss you off in ME3?

I want an ending that makes me cry. Maybe out of happiness, maybe out of sadness, but I want it to be emotionally powerful like the end of ME2.
A lack of tense moments would ****** me off. I want the possibility of squad members dying, so that decisions actually feel important.
A lack of conclusion to the romance subplots. If at the end of the game Shepard and Garrus are alive and well, I want even just a short cutscene showing that there is some continuing romance, and the same for the other pairings.
Picking a new squad, as others have pointed out. Maybe a few new characters, but don't throw out the old.
Lack of consequences for previous actions.
Squadmates getting nothing more than cameos, like Liara, Kaiden, and Ashley in ME2. This is the last game, everyone should play a part.

No helmet toggle.
Brillant lol!!!!!!!
If Shepard was really a clone all this time, and/or now because he has made TIM angry he has to fight a Shepard clone, because Cerberus thought in advance.
If TIM was a Reaper
If Liara died
If we didn't get to see Tali's face ( I wouldn't be that mad, because somehow I know we'll never see what Quarian's look like. I think that's part of them being Quarians)
If Captain Anderson was really evil or he died.
Multiplayer
Less side quests
No clear Reaper or Prothean explanation whatsoever.
The Return of Saren.
Overall somehow I'm confident the game will be great. Didn't the developers say they were working on part three (or was it the story) when ME1 was released.

I definitely want an emotional game - but I hope it's a lot more intense than ME2, given that so much more will be at stake. I hope for the ending they bring it to the same level as "The Fountain"... And if a squaddie dies half way through the game, then I hope, just to give it more impact, that they get their own cutscene, like Asuka in End of Evangelion, or Boromir in LOTR...

Hmm if the whole story turned out to be some new recruits dream. If Bioware didn't bother to explain why the final enemy in ME2 was what it was.. (trying not to put in a spoiler lol). The geth and quarian war isn't resolved.
And before you all say "Well bioware did explain why the final enemy was what/who it was." Yes Edi tries to explain this, but in the end video when you see reaper fleet all moving out on the milky way galaxy ever reaper shown looks like Sovereign. So why? they totally contridicted themselves, and i hope they didn't do it because they were just lazy and not thinking.

@Normandy30, it could be that inside each reaper ship they look like the species they came from. That human reaper was nowehere near the size of a full reaper ship, so it's reasonable to assume that it was supposed to fit inside a cuttlefish-like reaper hull.

kicking most of the team out for new random people, more random LIs, that would suck

Past decisions to not have consequences or not even be included to aide newcomers to the story would ****** me off the most.....oh wait.

Multiplayer and I'd probably get a little annoyed too if Bioware didn't concentrate more on romance relationships

I agree with SoulfulStarfish. With the war becoming more intense, that is when people either break, or become closer together. Romantic relationships should be further explored. ME3 should be emotional, more intense, and make your heart race; and the words coming out of my mouth should be: "Oh shi-...WHAT DID I DO?! NO! LET ME GO BACK AND FIX IT!!"

If there was no explanation of the origin of the Reapers.
Not necessarily...
If Bioware went back to playing up the lovecraftian elements of the Reapers in ME3, it would be perfectly acceptable never to reveal their origins.
That'd be rather hard to do though, considered how much villain decay the Reapers suffered in ME2.
