I was working on the achievement for beating the game on insanity mode, but the game automatically switched to casual mode. I started on insane mode, but then the game switched to casual mode somewhere around the time where you recruit Mordin. I though I was doing really good, but I realized it was way too easy when I was destroying krogans with great ease on my way to get grunt. Is there anyway I can prevent this glitch from happening? I did not play on any other profiles, it may have happened after my game crashed...

Well this has only happened to me twice... onyl twice so not that major though.
If ur worried that you played and want the achievement, download the modding kit. Do not MOD anything if your the type that "doesn't want to cheat. There is a box that says "changed difficulty" uncheck it and the game will think you didn't change the difficulty.
Now have I ever tested it... no. I just assume thats what the box is for.
