Starting from the most annoying - LOKI on Hane-Kedar facility (plant with endless amount of them). On Insanity. With someone other than Sentinel.

2nd - THAT pyro (Garrus recruitment). Although with Infiltrator it was child's play

3rd - Praetorian at first time using Vanguard. Shepard has the total justification for weird dialogue lines on Horizon.

4th - original ME1 husks. Especially jumping on you at the narrow corridors at abandoned ship.

5th - Tela Vasir. It's too damn long, exclude maybe Vanguard who has some fun

Rachni and Husks. Especially the Rachni when they crawl up out of nowhere.

Any humanoid that couldnt master more words than "go go go!" and "i will destroy you". The dull repetativeness of it made me wish i was fighting husks, on insanity, with nothing more than a rolled up newspaper.

For the combat:

(1) YMIR mechs. Take forever to take down and I end up with no ammo.

(2) Scions. Especially if accompanied by Husks (i.e. always). Damn Shockwaves that push me out of cover.

For the groan effect:

The human Reaper. Ugh. Cheesy horror pushed up to eleven. Bad, bad justification for using humans. Destroys my suspension of disbelief and makes the ME universe shatter around me in my mind.

yes, the flamer vorcha behind that corner in garrus mission is the most annoying enemy in the game. I cannot believe this passed the QA play testing.
