Import the last save you made with the character whose story you want to have affect your world in DA2. So if you took a character all the way through from Origins, imported into Awakening, imported into Golems, imported into Witch Hunt, then you use the auto-save Witch Hunt creates at the end of the game. If you only did Origins and Awakening with the same character, then use your last Awakening save.
The import process from one module to another keeps the saved data from the previous game. Assume you have a character you imported from Origins into Awakening into Golems. That's how Jerrik for example knows which King you put on the Orzammar throne because that information was still in the save brought over from Awakening even though that had nothing to do with Awakening.
If your Warden performed the Ultimate Sacrifice in Origins and you still imported the same Warden into Awakening, the game will still set the flags for your actions in Awakening/Golems/Witch Hunt but treat all that as being done by an Orlesian Warden. Your character will still be considered dead.
The only issue that comes up is if you created a new Orlesian Warden for Awakening/Golems/Witch Hunt. Since this is another character, you'll then have to choose which to import - the original Warden with the Origins game flags set and use the defaults for the rest or the Orlesian Warden with the Awakening+ game flags set and the defaults for Origins (whatever said defaults may be.) Reason being is that it is a separate character with a separate story and more importantly, its own set of saves.
But if you had simply been transferring a Warden from 1 module to the next, just use the last save created from the last module played. That's all.
