Dude on the left looks like Kaidan in Horizon armor to me. In fact, it looks ripped directly from Horizon to me. Judging by the placement and other camera angles, it could be Thane, but no trenchcoat and the glowy weapon-clip-on spots appear visible for one frame.
But then, I may be seeing things that aren't there. Ashley is there, right? Someone tell me Ashley is there and I'm not making stuff up to make myself feel better....

I really do not want the dead person from Virmire brought back magically. It cheapens a heart wrenching part of the first game, and all of the whiners who passionately hate Ash or Kaidan or both will complain loudly.
Is it just a vision that Shepard is having at the last moment of his/her life? Why wouldn't all of the ME2 squad be there? SO CONFUSED

Maybe it's a secret hint for ME3...in all honesty I wouldn't mind having that line-up for my ME3 crewIMO the important people are there, Alliance represented...Cerberus...and the fan favourites

All ten of the normal ME2 squadmates are there in one analogous shot with no one else (no Kasumi or Zaeed). There's also a shot from that front of that group there, which includes Ashley, tIM, Miranda, Mordin, Garrus, and... uh... psh. *checks* Chakwas.
As for what it means... search me, I just play with deconstructing files. (Someone mentioned you only get it if you fail and the Reapers arrive, so I guess they mindblast Shepard and it's her vision while dying? Yeah, could be ChakwasAnd those not in that shot could still be in the game, but with cameos. Hence why I said it seemed logical to me since those not showing up could easily have gone back to whatever it was they were doing before Shepard came along

edit : No doubt Kaidan is in that shot along with Ash. Even has the blinking lights on his back. I agree with Chig, one scene to fit all no doubt

That scene is obviously the big musical number at the end, where all the characters come back to sing, regardless of whether they died during the show or not.

Not much.
It's in the Arrival files. Some people say it occurs when you let the time run out; I suspect that you have to let the Reapers arrive in order to see it (rather than simply die/die while trying to escape). Ashley is in it. Kaidan is... kinda in it, maybe, sort of, from the back, copy and pasted?
